Clinical Nurse Specialist / Clinical Paramedic Specialist

Main duties of the job

Your new role

As part of our model of care to ensure we have the right people, providing the right care in the right place and at the right time, we are offering Clinical Nurse Specialist (CNS) or Clinical Paramedic Specialist (CPS) roles, which are the equivalent to a Band 7, to work as part of our multi-disciplinary team within the Hospice Outreach Service in the community.

So, what does that mean? Our Hospice Outreach Team are the initial point of contact for all new community referrals and all telephone calls coming into the service. They are at the forefront of providing palliative advice and support to patients and families in the place they call home, including care homes and nursing homes. You will be part of a rota of clinicians covering our seven day a week service offering telephone support, as well as clinic and home-based assessments.

Our team care for patients who have a life limiting illness and our focus is on individualised care. We focus on improving quality of life and achieving patient centred goals. We care for people with long term conditions; including heart failure, respiratory disease, frailty and neurological conditions, as well as those with an oncological diagnosis.

At the Hospice, our team strive to work alongside other community teams and specialists to optimise patient experience and improve our team knowledge and development.


About us

To succeed in this role...

You are a motivated clinician with the ability to work as an independent practitioner, while also being part of a responsive and resilient team. You possess advanced communication skills and excellent presentation skills (both oral and written).

You have proven community or in-patient palliative care experience at Band 7 or have the equivalent level of experience working in the following related areas: Paramedic, General Medicine, Neurology, Emergency Medicine, Frailty, Respiratory Care, Cardiology or Oncology. You will have sound pharmacological knowledge of medications used within the specialty of palliative care /EOL care.

Based at the Hospice buildings in Pembury, Kent, or FiveAshes, East Sussex, with potential requirement to work at other locationswithin West Kent and East Sussex.

Scope& Job Purpose

Youwill co-ordinate and deliver high quality care to patients with a terminalillness. Providing support and adviceto patients in their own homes, community hospitals and care/nursing homes .

You will strive to help us todeliver Hospice advice and support to more people who need it, you will provideclinical advice and interventional support to other professionals onpalliative care issues and play asignificant role in helping patients to achieve their personal goals and preferredplace of care.

KeyResponsibilities

Effectively and efficientlymanage patients, ensuring the right person is seeing the right patient in theright place at the right time.

Work as part of a team toachieve referral outcome measures as stated in the Hospice in the WealdBusiness Strategy.

Attendance at palliative care meetingsheld at GP practices.

Engage in and promote the use ofEnd of Life Care tools.

Participate in the provision of high-qualityinformation to patients and those important to them regarding their disease,treatment and services available; adapting this advice to suit individual needs.

Undertake the Advanced PhysicalAssessment of patients, ensuring a holistic assessment is completed so that,as an organisation, we can offer the best possible care for the patient and thoseimportant to them

Ensuresymptom control and End of Life Care is delivered, documented and reviewed tothe highest standard.

Ensurethat the patient is reviewed at MDT or caseload review in accordance withNICE Guidance

Participate in the provision ofa 7 day a week service including weekends and Bank Holidays. Participate inagreed methods of duty planning.

Management and Leadership

Liaiseclosely with line manger on control, distribution, and management of clinicalworkload.

Be cognizantof the Hospice in the Wealds Three-Year Strategy and Operational BusinessPlan.

Work to maintain a high level of risk awareness aboutself and patient safety

Assist in the development of policies, procedures, and guidelines tocontinuously improve ways of working.

Support and empower our volunteers and morejunior members of the workforce

Communications and Teamwork

Advise and support the multi-disciplinary team toenable them to provide high quality care, planning ahead, giving advice onpain and symptom control, advanced care planning, social and psychologicalcare, communication and use of resources.

Provide a high standard of sensitive communication withpatients and those important to them, which maintains an individuals privacyand dignity.

Auditand Research

Inconsultation with managers, ensure all work is evidence based.

Contributeto performance and activity reports and analysis to Hospice in the Wealdusing ICT/information to produce intelligence for future service provision.

Develop,in conjunction with line management, mechanisms for auditing care within theservice and implement changing practice as a result.

Participatein or support, where appropriate, nursing and/or collaborative clinicalresearch in palliative care

Trainingand Development

Activelyplan and participate in the provision of supervision/reflective practice tocolleagues to support engaging development opportunities.

Undertake Hospice mandatory training and participate inwider training and development opportunities

Actas a role model and mentor to colleagues and to be an ambassador to neighbouringhealth and social care providers, providing informal education and support tostaff in their provision of palliative care.

Workcollaboratively with the multi-professional team to provide training to carehomes on End of Life Care as required.

Undertakeexternal training to enhance the CNS/ CPS role as appropriate

General

Maintain professionalismthrough adherence to recognized Codes (e.g. NMC, GMC, HPC, HCPC) and workwithin the boundaries of Hospice in the Weald policies

Maintain professionaldevelopment though the Hospice Annual Review process to continuously updateand improve knowledge and competencies

Engage in the use of informationand communication technology (ICT) in the area you are working and achievethe required level of competence.

Any other duties that may bereasonably requested.

Hospice in the Weald is a registeredcharity and not part of the NHS. We rely on support from the community weserve, to enable us to develop and deliver our services. It costs over £7million to run the Hospice every year the majority of which comes fromvoluntary contributions from the general public. In addition, we have over1000 volunteers giving their time to the Hospice by helping in every Serviceand Department.

We expect all members of staff to supportthe fundraising activities of the Hospice. We encourage every staff member tohelp out at, at least two fundraising activities every year. Opportunities tosupport fundraising activities are varied, and could include helping atfundraising or shop events, attending a cheque presentation or giving a shorttalk about the work of the Hospice to an interested group in the community.

Your manager will ask you which events youhave helped out at, when you meet for one to one meetings and as part of theAnnual Review process.


Person Specification


Qualifications

* Registered Nurse (Level 1) or Registered Paramedic (HCPC)
* with 3 years post registration experience.
* Evidence of ongoing Professional Development.
* Principles of End of Life Care masters module.
* Experience Proven track record of contemporaneous clinical practice.
* Experience in Palliative and End of Life Care and Bereavement Support in a community setting
* Prepared and delivered education programmes to other healthcare professionals.
* Ability to motivate and inspire others to achieve in a fast paced and challenging environment.
* Takes the initiative and responds flexibly to change.
* Delegates tasks appropriately.
* Ability to develop rapport with a wide range of people and to act with credibility, tact and diplomacy.
* High level of computer literacy skills including the use of mobile technology.
* Ability to interpret situations from different angles and identify opportunities for improvement.
* Forward thinking and proactive in decision making and in assessing risks and its mitigation.
* Applies personal organisation strategies to prioritise and effectively manage workload within a team environment.
