<div><p>We are seeking a motivated Clinical Fellow to join our Frailty team, supporting the delivery of high-quality, patient-centred care for older adults.</p><p>The post holder will work closely with a team of Specialty Doctors and Consultants within a skilled multidisciplinary team. You will play a key role in the assessment, diagnosis, and management of frail and older patients presenting to the Emergency Department and subsequently admitted to the Frailty Same Day Emergency Care (SDEC) service and Frailty Decisions Unit.</p><p>This role offers an excellent opportunity to develop expertise in frailty medicine, with a strong focus on comprehensive geriatric assessment, early decision-making, and multidisciplinary working.</p><p>The primary aim of the service is to provide timely, safe, and effective care that supports patients to return to their usual place of residence as soon as possible, avoiding unnecessary hospital admission wherever appropriate.</p><h3>Responsibilities</h3><ul><li>Participate in Consultant-led ward rounds within the Frailty Decisions Unit.</li><li>Identify patients within the Emergency Department who are suitable for assessment and management within the Frailty Same Day Emergency Care (SDEC) service.</li><li>Undertake Comprehensive Geriatric Assessments (CGA) as part of a multidisciplinary team, under the supervision of the Consultant.</li><li>Deliver high-quality medical care in line with agreed clinical management plans.</li><li>Facilitate safe and timely discharge of patients, including completion of accurate and prompt clinical documentation and communication with General Practitioners to ensure continuity of care.</li><li>Ensure discharge plans are effectively coordinated with community health and social care services to support ongoing patient needs.</li><li>Support the training and supervision of Foundation Year (F1/F2) doctors under the guidance of Specialty Doctors and Consultants, and participate in teaching activities including Grand Rounds.</li><li>Participate in clinical audit, quality improvement, and governance activities as agreed with the Educational Supervisor.</li><li>Maintain compliance with data protection requirements and information governance standards.</li><li>Adhere to all relevant Trust policies, procedures, and clinical guidelines.</li><li>Undertake additional duties in response to clinical emergencies or unforeseen circumstances as required by Consultants or Specialty Doctors. Any such duties will be temporary and not of a sustained or regular nature.</li></ul></div>
