Community Band 6 Clinical Nurse Specialist in Palliative Care


Location

Little Hulton, Salford

£39,959 – £48,117 pro rata, Hospice Band 6, Competency Based Pay Framework


Hours

30 hours per week, incorporating working weekends and BH’s.

Usual working hours 9am-5pm, however, working 8am-6pm will be required to cover the urgent response service on average 1 or 2 times per week.

Job Summary


The Role

The Community Band 6 Clinical Nurse Specialist plays a key role in delivering high‑quality specialist palliative care to patients and those close to them within a community setting.

You will support the assessment, planning and delivery of holistic, patient‑centred care, working closely with the multidisciplinary team to ensure coordinated and responsive care provision.

The role involves maintaining accurate patient documentation, contributing to effective communication across services, and supporting the day‑to‑day coordination of care delivery. You will act as a specialist resource for patients, carers, and healthcare professionals, and contribute to education and service development initiatives.

You will also:

* Undertake holistic assessments and develop care plans for patients and those close to them
* Support delivery of a 7‑day community palliative care and urgent response service
* Facilitate sensitive conversations, including advance care planning discussions
* Contribute to clinical governance, audit, and data collection to support service improvement
* Promote strong working relationships across primary, secondary and tertiary care partners
* Support learners and contribute to the team’s education strategy


Requirements

We’re looking for a compassionate and motivated nurse who can demonstrate:

* 1st Level Registration with evidence of continued professional development
* Experience working at Band 5 level or above, including experience in palliative care
* Ability to manage a caseload with support from senior colleagues
* Excellent communication and interpersonal skills, including the ability to support sensitive discussions
* Ability to work both autonomously and as part of a multidisciplinary team
* Commitment to evidence‑based practice and ongoing learning
* Understanding of clinical governance and current issues in palliative care
* Strong organisational and time management skills
* Hold a full driving licence and have access to a vehicle insured for business use as the role requires travel between patients’ homes throughout the day
* Community experience
* Advanced communication skills
* Willingness to complete Non‑Medical Prescribing qualification
* Experience of audit or research activity

The successful candidate will be subject to a DBS Check at the level appropriate to the role, the cost of which is covered by the hospice. Any post holder within the organisation will be expected to undertake safeguarding training appropriate to their role and adhere to safeguarding policies and procedures. All staff must work in accordance with their statutory roles and responsibilities in relation to safeguarding in accordance with the Working Together to Safeguarding Children 2023, The Care Act 2014, and Prevent Duty 2015.
