WHSCT_Macmillan Palliative Care Nurse Specialist Band 7
To work within the Multi-Disciplinary Specialist Palliative Care Team (consisting of medical, nursing, allied health professionals and social work) to provide a specialist palliative care service for patients with malignant / non-malignant disease and their families/carers who have specialist palliative care needs. Be responsible for the assessment of care needs, the development and evaluation of individual programmes of care, and the setting and auditing of standards of care for patients with specialist palliative care needs. To provide specialist advice, support and education / training to other staff, patients and their families and carers. To work in close collaboration with other Health Professionals in the statutory and voluntary sectors to nurture care programmes, in order to provide a seamless service.
Responsibilities
* Establish effective working relationships with the Palliative Care Team and with other members of the Cancer Team and long term conditions.
Skills and Qualifications
* Registered Nurse on Part 1 (Adult) of the live NMC Register.
* Specialist Practice Degree in Palliative Care OR currently undertaking and near completion of this course. (No job offers will be made until qualified in specialist practice and registered with NMC).
* 3 year’s post-registration experience caring for patients with cancer/palliative care or haematology needs in a health care setting.
