This is a fast paced, exciting role, for a practitioner who can manage change effectively and demonstrate your ability to be resilient under pressure. You will need to be confident and competent in assessing risk, identifying appropriate need and have experience of providing social care/ multidisciplinary interventions. The ability to work effectively in a team, commitment to customer service and excellent communication skills are essential. In person assessments and reviews in the community we serve are an essential part of the role therefore an ability to meet the travelling requirements of this post will need to be demonstrated. The ideal candidate will have: A wide range of Safeguarding experience and an ability to work independently and part of a team In depth experience of risk assessment and complex case management work A good knowledge base and understanding of completing assessments under the Care Act 2014 and working within the Mental Capacity Act 2005.
