Role Overview

Act as a proactive clinical leader and a role model of expert clinical practice and deliver clinical care across the Urgent and Emergency Care footprint according to local and national guidelines to ensure delivery of evidence based care.


Responsibilities

* Provide advanced levels of practice, knowledge and skills to promote inspiration and development of the Advanced Clinical Practitioner role.
* Undertake mentorship and clinical supervision of staff, assessing competence against local, regional and national professional and clinical standards.
* Document clearly all aspects of assessment, diagnosis, and management plan, presenting findings using approved documentation framework.
* Participate and contribute to Advanced Practice and specialist forums, locally, regionally and nationally.
* Critically appraise outcomes of relevant research, evaluations and audit and disseminate evidence‑based practice and audit locally, regionally and nationally via relevant forums.
* Work within Adult and Children's Emergency Departments, Acute Medicine areas and Urgent Treatment Centres, supporting the development of the Advanced Clinical Practitioner service across Emergency Medicine.
* Coordinate the care of Emergency Medicine patients throughout their pathway with the multidisciplinary team and clinical services to ensure effective management.
* Lead and deliver clinical care across a variety of settings according to national and local guidelines, ensuring delivery of evidence‑based care.
* Undertake initial assessment to determine differential diagnosis, utilising advanced critical thinking and decision‑making skills, deciding when necessary to refer to senior clinical colleagues.
* Act autonomously to assess, prioritise and provide expert clinical care, contributing to the patient’s treatment plan to ensure optimum physical, psychological and social care needs are met.
* Request and interpret relevant investigations to facilitate the development of the patient’s management plan.
* Proactively refer to other clinicians or services depending upon patients’ needs to deliver care beyond your limitations and scope of practice.
* Proactively initiate treatment plans for patients requiring intervention and treatment, ensuring ethically based considerations.
* Proactively identify, diagnose and manage treatment plans for patients at risk of developing long‑term conditions.
* Undertake relevant clinical procedures to facilitate patient care within the service, supported by the appropriate training and supervision.
* Autonomously assess, diagnose, prescribe (as per scope of practice and professional regulatory requirements) and review medications for therapeutic effectiveness in the context of acute/emergency illness or chronic illness.
* Provide information and advice to patients on prescribed medication regimens, side‑effects and interactions, ensuring compliance with evidence‑based practice and national practice protocols and within their scope of practice and professional regulatory body requirements.
* Integrate pharmacological and non‑pharmacological treatments into patient management plans as appropriate.
* Maintain highly developed communication skills enabling maintenance of high‑quality service within a complex and challenging environment with often conflicting priorities.
* Apply safeguarding measures that support the identification of vulnerable adults and children; be aware of statutory child/vulnerable patient health procedures and local guidance.
* Undertake delegated consent after successful completion of relevant training and competencies.
* Report clinical incidents via Trust Datix reporting system.
* Use SBAR to communicate clinical concerns to senior medical staff when a patient is critically unwell or deteriorating.
* Communicate complex information effectively to patients and carers, recognising and respecting diverse communication needs.
* Maintain effective communication with the multidisciplinary team and other services, providing constructive feedback as appropriate.
* Demonstrate advanced communication skills in breaking bad news and supporting patients, family and staff.
* Act as an advocate for patients and colleagues.


Qualifications & Requirements

* Qualified Adult or Paediatric Advanced Clinical Practitioner with advanced critical thinking in clinical decision‑making.
* Demonstrate ability to work collaboratively with Emergency Medicine teams and be key to shaping the Advanced Clinical Practitioner service.
* Hold an MSc in Advanced Practice and provide a portfolio of evidence demonstrating the scope and standard of practice in Emergency Medicine, utilising NHS capability frameworks and RCEM credentialing documents.
* Provide an immunisation and vaccination report from a UK occupational health provider or GP.
* Licence to prescribe, as per scope of practice and professional regulatory requirements.
* Strong communication, safeguarding, leadership and multidisciplinary working skills.
