Overview

An exciting opportunity has arisen for a committed, competent and passionate team leader to join our District Nursing Teams within the Community Division for patients across LPB. Mersey Care is a well-established organisation driving forward a transformational programme and embracing a Just and Learning Culture and its principles and values.

The District Nursing Service currently operates an 8am-8pm working model with shifts across 7 days a week to support the needs of the service. The post holder will operationally manage and lead a District Nursing Team, accountable for performance, quality, and clinical effectiveness, and will provide direct patient care when necessary as an expert practitioner and role model. The role requires working closely with the Integrated Community Nursing Senior Leadership Team to develop a high-performing team and drive service improvements that meet patient need while promoting staff experience and development.


Responsibilities

* Lead and manage a District Nursing Team, ensuring the team understands and owns quality, recognises patient safety concerns, and identifies, assesses, and manages risk.
* Provide direct patient care when necessary and act as an expert practitioner and role model to the team, supporting integration within the service.
* Collaborate with the Integrated Community Nursing Teams and the Senior Leadership Team to develop an effective and high-performing team, driving service improvements.
* Promote a positive work environment and staff development within the Community Division.
* Lead clinical delivery in line with NICE guidelines and relevant legislation within own scope of practice.
* Ensure community nurses practice within legal and ethical frameworks, adhere to The Code, and comply with local Trust policies and procedures.
* Maintain confidentiality and communicate patient information to preserve dignity and privacy.
* Promote non-discriminatory practice and obtain informed consent before acting.
* Act as an advocate for individuals and groups and escalate professional issues as needed.


Care/Caseload Management

* Coordinate care for patients with long-term conditions, disease management, and ongoing support for clinical staff to ensure continuity of care.
* Develop an annual caseload profile to identify health needs and resource requirements, informing service development.
* Set team objectives in collaboration with the Operational Lead and ensure adherence to Human Resources policies, including recruitment and sickness absence management.
* Ensure accurate data entry of patient activity and act as an authorised signatory for budgets and procurement where applicable.
* Allocate work to maximize team skills and ensure appraisals and Personal Development Plans are completed.
* Identify and mitigate risks to staff, patients, and service delivery; lead accident/incident reporting and engage users to shape services.
* Lead by example, challenge current practices constructively, and support staff through times of change.


District Nurse Team Leader Role

* Promote optimum health for patients with long-term conditions through predictive and proactive case management of an identified caseload.
* Formulate care plans addressing health, social, and cultural needs in partnership with patients, GPs, specialist nurses, and other providers.
* Promote patient-centred care by coordinating activities among patients, families, carers, and teams to manage long-term conditions effectively.
* Ensure information about the patient’s condition is shared with GPs and relevant stakeholders through effective inter-agency communications.
* Provide clinical leadership to nursing teams to address complex needs, liaising with Social Services and GPs to support transitions between primary, secondary, and tertiary care.
* Contribute to policy development for long-term conditions and disease management and promote user involvement in shaping services.
* Deliver services in line with NICE guidelines and local targets, leading, motivating, educating, and developing colleagues.
* Promote admission avoidance and early discharge through effective liaison with stakeholders.


Personal/Professional Development

* Participate in setting personal objectives with the Operational Lead and engage in appraisal and clinical supervision processes.
* Provide advice on professional issues in community nursing and manage changes in ways of working.
* Identify and address poor quality practices, comply with community division objectives, and maintain ongoing professional knowledge and skills.
* Support induction for new staff and contribute to policy development for primary/community care and district nursing.
* Maintain nurse prescribing competencies and ensure identification and reporting of risks to service delivery.
* Maintain quality standards of care using audits and monitoring systems.
