Job Overview:
We are seeking a Band 6 Occupational Therapist to join the Urgent Therapy Team in Central & South Liverpool. The role involves providing rapid assessment and intervention to prevent hospital admissions and facilitate timely discharges for patients with complex needs.
Key Responsibilities:
Deliver urgent occupational therapy interventions in community and hospital settings.
Conduct rapid assessments to support hospital avoidance and early discharge.
Work collaboratively with multidisciplinary teams, including physiotherapists and social care.
Develop individualized care plans to improve patient independence and well-being.
Maintain accurate clinical records in line with NHS guidelines.
Key Requirements:
HCPC-registered Occupational Therapist with Band 6 experience.
Experience in urgent care, hospital discharge, or community rehabilitation.
Strong clinical reasoning and decision-making skills.
Ability to work autonomously in a fast-paced environment.
Car driver with access to a vehicle for community visits (preferred).
