Closing: 24/06/2026 23:59</p><h3>Unplanned Community Staff Nurse</h3><h3>NHS AfC: Band 5</h3><h3>Job overview</h3><p>The Unplanned Community Nursing Team provides a 7‑day a week 24‑hour service across the borough of Doncaster, delivering nursing care to patients in their own homes. We offer a supportive environment with training, partnerships with universities and a 12‑month preceptorship for new staff.</p><p>Our approach treats the whole person, enabling patients to stay at home longer and leave hospitals faster. Community nurses work closely with multidisciplinary teams including GPs, hospices, Clinical Lead Nurses and specialist nurses.</p><h3>Main duties of the job</h3><p>Assess, plan, implement and evaluate nursing care in patients’ own homes and over the telephone. Use excellent communication, IT, and telephonic assessment skills to refer patients into Community Nursing and coordinate appropriate services.</p><ul><li>Set, maintain and deliver high standards of care at various locations.</li><li>Under the supervision of a Senior Nurse, triage incoming referrals, assess patient needs and determine priority of care.</li><li>Apply a multisystem approach to case management, liaising with case managers and the wider health and social care community for a seamless patient journey across Doncaster.</li><li>Provide first‑line management of junior staff, offering support, mentorship and acting as a positive role model while maintaining standards and best practice evidence base.</li></ul><h3>Person specification</h3><h3>Qualifications</h3><ul><li>Registered General Nurse</li><li>NMC Registered</li></ul><h3>Experience</h3><ul><li>Experience working with other professional disciplines</li><li>Experience working both as part of a team and autonomously</li><li>Evidence of effective team working</li><li>Community Nursing experience</li></ul><h3>Skills / Abilities</h3><ul><li>Work with minimal supervision</li><li>Provide evidence‑based care responsive to patient needs</li><li>Use own initiative and make decisions</li><li>Excellent verbal and written communication skills</li><li>Team player</li><li>Experience teaching and assessing staff</li><li>Coping with change</li><li>Prioritise own work and that of the nursing team</li></ul><h3>Knowledge</h3><ul><li>Effective clinical skills</li><li>Move and handle patients according to the Organisation Manual Handling policy and maintaining sufficient mobility for a community role</li></ul><h3>Eligibility for sponsorship</h3><p>We welcome applications from individuals who may require sponsorship under the skilled worker or health & care visa route.
