Overview
An exciting opportunity has arisen to join the Integrated Community Care Team (I-CAT) based in Helensburgh. The successful candidate will join a busy and highly motivated team and will provide leadership and management responsibility to the team. A good knowledge of the government frameworks in relation to Prevention of Admission to Hospital; Discharge Without Delay and Anticipatory Care Planning would be required, in addition to experience of the Assessment and Planning of Care for people living in their local Community. This team forms part of a wider multi-disciplinary team consisting of District Nursing, Occupational Therapists, Physiotherapists, Reablement, Palliative Care and Social Work services. The successful candidate will work closely with all colleagues to provide leadership and managerial support resulting in a seamless and professional service to our community. The postholder should be a motivated team worker with experience of Community Care.
