Integrated Care Worker (Rehab) Development Role

We are offering a permanent contract in Ely. The Home First Intermediate Care Service supports patients to achieve optimum independence by earlier discharge from hospital and avoidance of hospital admission. You will assist in the delivery of high standards of health care and support patients within their own homes following prescribed care and therapy packages.
The vision of the service is to provide outstanding, patient‑focused, community‑based health care and therapy, optimising people’s potential to achieve their goals by supporting earlier discharge from hospital, early assessment for care needs and avoidance of hospital admission.
Operating hours: 7am to 10pm, 7 days a week.
Band & Development – This post may be offered as a Band 2 Development position depending on qualifications and experience. You will be supported fully to develop into a Band 3.
Requirements – You will be required to travel independently around the county meeting strict time deadlines, hold a full UK driving licence and have use of a vehicle. Public transport cannot be used.
Responsibilities
* With training, use skills from Nursing, Allied Health Professionals and Medicines Management to undertake delegated tasks.
* Apply competency‑based treatment techniques/care to patients with specific conditions under the supervision of a qualified practitioner, following a prescribed treatment/care plan.
* Provide care to patients with deterioration in their health or function with a focus on admission avoidance to acute hospitals.
* Deliver functional assessments, personal care, medication prompts, mobility and appropriate exercise, ADLs, equipment provision.
* Carry out duties delegated and supervised by registered professionals.
* Provide a high standard of care.
* Manage own caseload of patients who require ongoing management and support with complex needs.
Qualifications & Conditions
Band 2 Development role depending on qualifications and experience; full UK driving licence required. Closing of adverts may occur early if sufficient applications are received.
