Description\n\nWe are in search of a Hospital Social Worker or Care Manager. To assess the needs of individuals with complex needs, taking into account levels of risk and their strengths, to protect their rights and promote their independence and interests. The successful candidate will play a vital role in delivering services that promote independence, safeguard vulnerable adults, and support the well-being of our community.\n\nResponsibility\n\nTo offer professional guidance to people and their carers to enable them to make informed choices about the care services that they receive and to produce a person-centred support plan which promotes their independence, dignity, choice, self-esteem and well-being.\n\nQualification – Essential \n\nGCSE Grade (or equivalent) in English Language and Mathematics.\n\t\nDegree in Social Work) or a recognised Occupational Therapy qualification,\n\t\nor a relevant community Nursing qualification.\n\t\nSocial Work England (SWE) Number\n\t\nMicrosoft Office, Word, Excel, Outlook, social media and smartphone.\n\nEssential - Skills, Abilities, & Experience\n\nExperience working in hospitals or adult social care.\n\t\nStrong knowledge of the Care Act, Mental Capacity Act, and safeguarding procedures.\n\t\nExcellent assessment, communication, and multi-agency working skills.\n\t\nAt least 2 years’ experience in a Local Authority, Health or voluntary sector setting.\n\t\nKnowledge of relevant legislation and a fundamental understanding of, and commitment to, the principles of personalisation, independence and choice.\n\t\nExperience of working with older or physically disabled people and knowledge of service provisions for these groups.\n\t\nExperience of working in a multi-disciplinary way.\n\t\nExperience with assessment work.\n\t\nAn understanding or experience of budgets and financial controls.\n\t\nAbility to carry out accurate and timely assessments and promote self-directed care.\n\t\nAn ability to communicate clearly in writing, verbally and through reports with service users, carers and other professionals.\n\t\nAbility to work independently using own initiative, prioritise workload, and meet deadlines.\n\t\nAbility to negotiate on behalf of the Authority and/or the service user, with other agencies.\n\t\nAbility to work proactively, to pre-empt / and or prevent and manage a crisis.\n\t\nAbility to work as a team member in a multi-disciplinary service.\n\t\nAbility to keep comprehensive case records, which can be shared with service users in line with the Open Files Policy.\n\t\nAbility to undertake a comprehensive assessment of service users’ support needs.\n\t\nAbility to communicate professionally in a language other than English (desirable\n\nEssential Compliance Requirements\n\n5 Years References\n\t\nProof of Qualifications\n\t\nEnhanced DBS on the update service\n\nA pre-engagement screening is mandatory for this role.\n\nDiamond Blaque Group, a leading provider in the public sector, is proud to be an equal opportunity employer and acts as an employment agency for this vacancy
