What the Job Involves In this role, you will play a crucial part in our Reablement Team, working to empower individuals post-hospitalization to rebuild their confidence and independence. You will undertake professional assessments, design tailored reablement plans, and provide expert advice and training to team members. Your responsibilities include managing a complex caseload, engaging with clients and their families, assessing occupational therapy needs, and implementing innovative solutions to facilitate reablement. You will have the opportunity to work collaboratively with a network of healthcare and social care professionals to ensure that the reablement goals are effectively achieved. Your work involves traveling to clients' homes or other relevant settings, where you will apply your expertise to deliver compassionate, bespoke care and ensure the safeguarding of vulnerable adults.
