Advanced nurse practitioner/ paramedic

We are looking for an Advanced Care Practitioner (Advanced Nurse Practitioner / Paramedic) to join our established Home Visiting and Care Home team. The practitioner will provide on‑day acute visits for housebound patients and care home residents within the PCN, as well as deliver proactive home visits to support the ongoing health needs of our population. In addition, the post holder will provide on‑day visits for housebound patients who are acutely unwell and need a clinical assessment, and pre‑planned proactive care assessments. This will include visiting patients living in care homes for both acute and proactive care, in the form of either a ward round or an acute visit. The clinician will be responsible for prioritising visits according to clinical need and will be required to take a comprehensive history and devise a treatment plan, including liaison with the GP, secondary care and community colleagues to support coordination of patient care.

Responsibilities

The role includes:

• Performing acute and proactive home visits to housebound patients and care home residents.
• Prioritising visits based on clinical need.
• Taking comprehensive histories and developing treatment plans.
• Co‑ordinating with GPs, secondary care and community colleagues.
• Maintaining documentation and communication of patient care.

Qualifications

• Advanced Care Practitioner requirements: MSc, Current NMC Registration, Membership of a professional body, UK driving licence.
• Paramedic requirements: Educated to degree/diploma level in Paramedicine or equivalent experience, Registered with HCPC, Completed two‑year Consolidation of Learning period as a newly qualified paramedic, at least three years of experience as a paramedic, Independent Non‑Medical Prescriber qualification.

Experience

• Previous primary care experience in an Advanced Clinical Practitioner role.
• Own transport and driving licence to travel regularly between sites around West Southampton.
• Previous care home primary care experience.
• Experience of clinical audit, evaluation research and implementation of evidence‑based practice.
• Background in community nursing.

Salary: £58,000 – £61,000 depending on experience and qualifications.
