This is an exciting chance to help shape change as we become more sustainable as a charity. The postholder will work collaboratively within the Care Coordination Centre. The Care Co-ordination Centre is at the heart of Community Specialist Palliative Care services. It combines Rapid Response Team, Clinicians of the Day and the Triage and Deployment Team. Its role is to ensure that referrals are handled efficiently and patients are seen by the most appropriate clinician/practitioner for their needs. Advert You will work as a triage deployment nurse supporting the triage of all patients that are referred to the hospice ensuring they are routed to the most appropriate service. You will be supporting the use of our deployment tool working with the rapid response service ensuring that patients are allocated promptly based on need. You will also support the senior deployment nurse in the coordination of patients to be admitted to our inpatient unit. Detailed job description and main responsibilities We are looking for a nurse who has a keen interest in specialist palliative care with good clinical reasoning to support the triage and deployment of patients referred into our service. The successful candidate will have outstanding communication skills and be able to offer excellent customer service to service users and external professionals. You will be part of the Care Coordination Centre as a Triage Deployment Nurse on a permanent, full-time basis.
