Job Summary

Job Summary/Purpose To support individuals referred to secondary mental health services for assessment or ongoing care. To demonstrate the Trust values in inspiring hope, working together, to be caring and compassionate, to value and respect others/ourselves and to be open and honest. As a Registered Mental Health Nurse you will demonstrate and live the NMC values as identified in the relevant Code of Practice. Your post will include working with individuals using evidence-based interventions to promote their independence and recovery whilst maintaining high quality clinical documentation. This includes respecting individuality, values, cultures and religious diversity of clients and colleagues and thus contributing to the provision of a service sensitive to these needs.


Responsibilities

* • To promote positively mental and physical health to individuals under the care of the service in all situations.
* • To support individuals to collaborate in the development of their recovery plans and structured care plans and treatment, working to build a strong therapeutic alliance.
* • To assess, plan and review in line with requirements of the Care Act (2014) and s117 of the Mental Health Act (1983).
* • To provide clinical supervision to members of the multi-disciplinary team based on your clinical and professional knowledge.
* • To supervision junior members of staff in their clinical risk assessment and formulation of crisis and risk planning.
* • To promote recovery-focused care and interventions for individuals.
* • To ensure that all interactions and documentation is person-centred in line with NHS guidance ‘No decision about me, without me’.
* • To co-produce individualised recovery plans with the individuals, carer, and others identified by the Service User.
* • To promote and maintain a learning culture within the team which supports the professional and effectiveness of clinical care.
* • To contribute to the delivery of multidisciplinary training including the induction of new staff, students and trainees.
* • To always promote awareness of, and respect for, equality, diversity and dignity in care and work inline with Trust, and professional standards.


Practice Effectively

* • To demonstrate a high level of active communication skills with individuals in a variety of forums and situations.
* • To utilise your highly developed interpersonal skills to facilitate effective communication. This includes identification and overcoming communication barriers such as sensory or cognitive impairment, hostile or highly emotive situations.
* • To provide specialist mental health interventions in the assessment, support and recovery of individuals with severe mental health difficulties. These include but are not limited to Mental State assessments, Social Care Assessments, physical health care monitoring, side effect profile monitoring, etc.
* • To ensure that all individuals have clear and accurate information regarding their care or the care of their loved ones.
* • Where required to act up in the absence of the senior practitioner.
* • To seek out and participate in line management and clinical supervision.
* • To ensure, as far as practicable, the full involvement of individuals and their carers in the assessment, formulation and review of their care plans.
* • To maintain high standard of clinical recording ensuring that information is always accurate, appropriate and up to date.
* • To comply with Greater Manchester Mental Health NHS Foundation Trust’s policies and procedures with specific focus on confidentiality, Information Governance and professional practice.
* • To maintain your personal well-being using all available resources available to you via the Trust or professionally, including PAM assist, Occupational Health, etc.
* • To comply with all health and safety legislation and the welfare of yourself and the wider Trust.


Preserve Safety

* • To identify any unmet needs to support service improvement or planning to enhance individuals experiences of health services within the Trust or in the wider health economy.
* • To comply with the relevant NMC codes of professional conduct, local and national professional standards, policies and procedures.
* • To adhere to the principles of Safeguarding children and vulnerable adults inline with Trust policy and local and national reporting procedures.
* • With support from line managers, make safeguarding enquiries in line with Section 42 of the Care Act (2014) ensuring that safeguarding is person-centred, proportionate, specific and that outcome measures are achieved and risk is mitigated.
* • To ensure that colleagues and junior staff are competent and compliant with Safeguarding legislation and guidance.
* • To act inline with the Trust’s Infection Prevention and Control policies, procedures and best practice guidance to maintain high standard of Infection Prevention and Control (IPCC).
* • To comply with all service security requirements with regard to your own and others safety. These include lone working procedures, health and safety at work and risk management procedures.
* • To ensure the management of risk in accordance with the Trust Policies and procedures, including the investigation process in regard to Root Cause Analysis, complaints, accidents and near miss situations via the InPhases system.


Promote Professionalism and Trust

* • To uphold the Trust values and work in an environment free from the fear of harassment, discrimination, abuse and violence.
* • To promote the Trust’s Anti-racism agenda in all professional and personal interactions.
* • To maintain your personal and professional duty of candour where an incident or accident has occurred.
* • To develop and maintain good working relationships with all partners involved with individuals care or the wider health economy including General Practitioners, commissioners and third sector agencies.
* • To promote awareness of mental health and the role of the CMHT within local services, primary care services and also within the wider community.
* • To advocate on behalf of individuals under your care with regard to ensure that they are fully involved with their care and treatment.
* • To exercise excellent time management, punctuality, consistency and reliability within your role.
* • To demonstrate empathy and compassion to individuals or their family/carers who may want to make a complaint about their care.
* • To take responsibility for your own learning and professional development, making use of opportunities to expand your clinical, professional or governance skills.
* • To act as an effective and empowered role model for others sharing your knowledge base and clinical skills.
* • To act as a Practice Educator for students on clinical placements.
* • To always act with dignity and respect in keeping with the relevant NMC and Trust guidance.
* • To maintain your professional registration and to inform the Trust if there are any changes, conditions or restrictions on practice active on your registration.
* • To only use access to clinical information where required in the completion of your duties.
* • To maintain clinically sensitive information under the duty of confidentiality in line with GDPR and ensure that this is not communicated to anyone inside or outside without due cause (Principle of Need to know) and with the appropriate permission.


Trust Mandatory On-going Requirements

* • To undertake any other reasonable duty, when requested to do so, by an appropriate Trust manager.
* • To understand and comply with all Trust policies, procedures, protocols and guidelines.
* • To understand the Trust’s Strategic Goals and how you can support them.
* • To understand the need to safeguarding children and vulnerable adults and adhere to all principles in effective safeguarding.
* • To carry out all duties and responsibilities of the post in accordance with Equal Opportunities, Equality and Diversity and dignity in care/work policies and principles.
* • To avoid unlawful discriminatory behaviour and actions when dealing with colleagues, services users, members of the public and all stakeholders.
* • To access information, whether paper, electronic or in any other form or medium, only which is authorised to you as part of the duties of your role.
* • To not communicate to anyone or inside or outside the NHS, information relating to patients, services users, staff, contractors or any information of a commercially sensitive nature, unless done in the normal course of carrying out the duties of the post and with appropriate permission.
* • To maintain high standard of quality in corporate and clinical record keeping ensuring information is always recorded accurately, appropriately and kept up to date.
* • To ensure your day to day activities embrace sustainability and reduce the impact upon the environment by minimising waste and maximising recycling; saving energy; minimising water usage and reporting electrical faults, water leakages or other environmental concerns to the facilities department or their line manager.
* • To take reasonable care of the health and safety of yourself and other persons.
* • To contribute to the control of risk and to report any incident, accident or near miss.
