About Us
We are a two-site practice in Exeter with approx 8000 patients. Our team includes 7 GPs, 2 practice nurses, a trainee nurse associate and a Patient Services Advisor Team. As a Training Practice we have a GP Registrar and medical students.

Job Description and Responsibilities
* The Deputy Lead Practice Nurse will support the Lead Nurse with duties as delegated, including management of the practice nursing team (nurses, TNAs, HCAs, Phlebotomists).
* oversee clinical areas such as child and adult vaccination programmes, health promotion, chronic disease management, well woman and well man clinics, IPC; support the practice management team in reviewing and delivering clinical policy and procedure.
* Assist in implementing processes for managing patients with long-term conditions using evidence-based practice, including elderly and housebound patients.
* Senior member of the multidisciplinary team ensuring nursing services are delivered effectively to the registered population.
Main Responsibilities
* Patient Support and Clinical Services
* Develop knowledge and skills to deputise for the Lead Nurse in their absence and eventually lead the role.
* Infection Prevention and Control lead; implement audits, coaching, training and ensure policy compliance; produce IPC annual statement.
* Develop and embed care management programmes for frail/elderly patients; health promotion and wellbeing programmes; chronic disease management.
* Coordinate vaccinations, vaccination campaigns and immunisations; review medications for compliance; travel medicine provision; phlebotomy; chronic disease clinics.
* Maintain accurate clinical records; ensure coding and chronic disease registers are up to date.
* Lead in well woman/well man clinics; ensure chaperone policy compliance; assist GPs with minor surgery; respond to clinical emergencies; safeguarding; substance misuse policies; health promotion.
* Coordinate information technology and administrative protocols; manage nursing team rota; liaise with nursing and administrative teams; contribute to policy reviews and national developments.
Qualifications and Experience
Essential
* NMC Registered Nurse
* Experience in a primary care environment; experience as a practice nurse; experience in chronic disease management; ability to work autonomously; infection prevention and control experience; experience with Clinical IT Systems
Desirable
* Mentor/teaching qualification; postgraduate qualification in chronic disease management; nurse prescriber; infection prevention and control training; family planning qualification
* Travel medicine, spirometry, governance knowledge, public health knowledge
Skills and Personal Qualities
* Excellent communication, time management, teamwork and independent working ability
* Strong interpersonal skills, problem solving, IT competence, clear telephone manner
* Strong integrity, empathy, adaptability; commitment to professional development
Other Requirements
* Confidential handling of information and GDPR understanding
* Flexibility to work outside core hours; DBS check; Occupational Health clearance
* Nurse revalidation evidence; CPD as appropriate for the role
