An exciting opportunity has arisen for applications to the role of Team Manager for the Older Adult Home Intensive Treatment (HITS), Care Home Liaison and Single Point of Entry service in HMR. The team consists of four RMHN’s providing a 7-day HITS service from 9-5, two RMHN’s commissioned by HMR Local Authority to support care homes within the borough, and a Single Point of Entry (SPOE) service responsible for initial assessment and triage of all external referrals into older adult services in the borough.

Main duties of the job
* Oversee management of the HITS, Care Home Liaison caseload, and SPOE service, leading the nursing team and coordinating clinical and therapeutic care within the community:
o Manage quality of care provided to clients in the community and ensure supervised care at all times.
o Chair weekly clinical meetings.
o Set and monitor care standards, review care and treatment of patients, and maintain appropriate standards.
o Participate in developing care pathways to ensure optimal service delivery.
o Use evidence-based tools and interventions as appropriate.
* Carry a small caseload of patients and support colleagues with complex client management and joint work where necessary.
* Provide clinical leadership and robust decision-making to identify appropriate care pathways for service users.
* Work with the wider older people's service to operationalize a robust gatekeeping protocol.
* Liaise with ward and community service managers to ensure effective MDT working, prevent admissions, and facilitate timely discharges.
Working for our organisation
The Older Adults Home Intensive Treatment Service (HITS) is a small, friendly team offering an alternative to hospital admission for older adults experiencing acute mental health episodes. The service provides assessment and treatment at clients' homes, and works with inpatients on the older adult Beech ward to ensure timely discharge. The team shares a base with other HMR Community teams, enhancing support and collaboration.
