Social Worker - Hospital Team West Sussex County Council Location: St Richard's Hospital, Chichester Contract: Temporary Pay Rate: £38 per hour OR £35 per hour £150 per week expenses The Opportunity West Sussex County Council are seeking an experienced and compassionate Hospital Social Worker to join their dynamic team at St Richard's Hospital. This is a fantastic opportunity to make a real difference by supporting individuals during critical stages of their healthcare journey. Working within a fast-paced hospital environment, you will play a key role in supporting safe and timely discharges, promoting independence, and ensuring individuals receive the care and support they need. About the Role This is a hybrid position, with a requirement to be onsite 2-3 days per week at the hospital, alongside home working. You'll be part of a multi-disciplinary team, working across: Hospital wards A&E and the emergency floor Community settings post-discharge Key Responsibilities: Deliver a statutory social work service within hospital settings Undertake strength-based assessments and develop care plans Support discharge planning to maximise independence Attend ward rounds, family meetings, and MDT discussions Work collaboratively with partners across health, social care, and voluntary sectors Respond to referrals and complete assessments within required timescales Contribute to safeguarding enquiries where required Requirements SWE Registration Eligible to work in the UK Full driving licence & access to a vehicle Post-qualified Social Work experience Degree in Social Work (or equivalent) Why Connect2SocialWork?
