About The Role

We are excited to advertise for 2 Social Work Assistant Positions, to join our committed and highly valued staff team.
* The main role is to provide Care and support to Hounslow residents and carers and undertake Social Care Assessments and to identify their needs and desired outcomes which places the resident at the centre of all interventions and giving them choice and control over the delivery of services.
* To protect the rights and promote the interests of Hounslow residents and their carers to ensure they are kept safe, in line with the Council’s and Pan London Safeguarding Policy including contributing to the gathering information under supervision.
* To follow practices that comply with policies and procedures set by the Department and Unit, including standards and requirements set by the Care Act 2014, Mental Capacity Act, Pan London Safeguarding Procedures and other legal statutes as is appropriate.
* To work as part of a team to provide a high standard of care that meets the needs of Adult with mental health issues or diagnosis. To work within a framework which safeguards and promotes their well-being.
* To follow an assessment framework that encompasses implementation of a care planning and risk assessment structure, which will consider all the physical, psychological, emotional, mental and behavioural health needs of the client to support their independence in the community.
About The Team You’ll Be Working In
* The Adult Locality Mental Health Team (ALMHT) is committed to supporting adults with mental health needs to overcome their difficulties and aid their journey of recovery, where possible. The ALMHT recognises that adults with mental health needs face discrimination in their day to day lives and seek to understand the personal experiences of residents to understand their struggles and offer the right support at the right time.
* This includes recognition of adults with mental health needs from other excluded groups such as LGBTQIA+, Black and Minority Ethnic Groups and those with physical and/or sensory needs. ALMHT uses a Strength Based Approach and looks at what a person ‘can do’ rather than what they ‘cannot do’, focusing upon what outcomes the resident wants to achieve.
* The Adult Locality Mental Health Team (ALMHT), supports adults with mental health needs, focusing on recovery and overcoming difficulties. They address discrimination and use Strength-Base Approach to emphasise individuals’ abilities and goals. ALMHT collaborates with health services and community support to maintain mental wellness and prevent the need for formal social care.
* They also work with West London Mental Health Trust and North-west London Integrated Care Board- Hounslow for timely discharges and referrals.
* Our core mission is to respect adults’ rights, empower them to manage their mental health, maintain stability, and enhance their quality of life and independence.
Key Responsibilities
* To undertake Care Act assessments, Support Planning and Reviews.
* To undertake safeguarding duties as an enquiry officer.
* To participate in team meetings and other professional forums.
* Use a strength-based and preventative approach to your work and Intervention.
Your Skills
* Excellent communication skills, orally and in writing.
* Understand Safeguarding Adults and how to Make Safeguarding Personal whilst balancing risk.
* Ability to use research to inform your practise and intervention.
* Ability to work in a team and network with other partner agencies.
About You

* Knowledge in understanding the care of people suffering with mental illness, the Care Act, Mental Capacity Act 2005, Safeguarding adults and its processes.
* Ability to undertake responsibility for ensuring departmental policies and statutory regulations are observed. To work within the Department's safeguarding policy with the sole aim that the safeguarding and protection of vulnerable adults.
* The ability to record, report and disseminate significant information to the relevant professionals within the department and other professionals as appropriate to ensure the safeguarding and well-being of the vulnerable adult in our service. Ability to write clear and concise reports and communicate effectively with colleagues, partner agencies and other professionals.
* Ability to undertake key working responsibilities and have knowledge in devising, implementing & reviewing care plans / training programmes, evidencing outcomes, completing risk assessments that are person centred with regard to adults skills and abilities with a focus to maximise life opportunities and independence.
* At all times respect the confidentiality and the dignity of the client and others.
Essential For The Role
* An Enhanced DBS check is required for this role.
* Experience of working in social care setting.
* Good communication skills, orally and in writing.
