We have an exciting opportunity for a Senior Occupational Therapist to join our supportive, diverse and fun team.
We take a broad range of referrals, predominantly from the acute trust but also from community teams; stepping up vulnerable patients to prevent hospital admission. Our patients come with a wide variety of needs, from simple to complex and with a range of conditions and co‑morbities ranging from orthopaedic and neurological to ICU step downs, giving you the opportunity to think outside the box and enhance knowledge and skills.
The Occupational Therapy Team pride themselves on their ability to innovate and adapt to the challenges of rehabilitation with active quality improvement programmes, to ensure that practice is current, and the patient voice is heard throughout.

The Team consists of Occupational Therapists, Physiotherapists, Nurses, Doctors, Consultants, Social Workers and Healthcare Assistants who work together in an integrated way to provide co‑ordinated, clinical support to promote independence and to improve quality of life, to provide advanced clinical and professional Occupational Therapy to the Community Therapy Team/Community Hospital Team, and to provide support to the Integrated Community Care team leaders in ensuring that the team contributes to the implementation of Hampshire & Isle of Wight Healthcare Clinical Governance Strategies.
Responsibilities
* Undertake Occupational Therapy assessments of patients with diverse and complex conditions, providing a clinical diagnosis (where appropriate), development and delivery of individualised treatment programmes.
* Support the case management process by providing expert clinical advice and therapy to patients with a variety of complex conditions living in the community who are at risk of deteriorating health that may result in declining quality of life or avoidable hospital admission to secondary care.
* Provide clinical Occupational Therapy skills within the Community Therapy Team/ Community Hospital Team(s) and all other provider organisations.
* Be skilled in assessing and treating a range of complex factors relating to the patient's condition, diagnosis and prognosis.
* Provide specialist intervention tailored to meet the uniqueness of the individual (e.g. complex manual handling and 24‑hour posture management).

Across Hampshire, our community hospitals provide inpatient rehabilitation as a step down from acute care, and our dedicated teams also staff Treetops Sexual Assault Referral Centre in Portsmouth, offering expert, compassionate support.

£39,959 to £48,117 a year. Please note for part‑time hours the salary will be pro rata.
