We have an exciting permanent, full-time (37.5 hour) opportunity for a new member to join our multidisciplinary team with the Stroke Early Support Discharge team as a Band 3 Rehabilitation Assistant. We are looking for a kind, passionate, organised, and resourceful individual to join our welcoming team to provide stroke specialist rehabilitation. You will be joining an experienced and supportive team striving to meet the physical, communication, cognitive and functional needs of patients following a stroke within the community.

Main duties of the job

* To provide stroke specialist rehabilitation in the community.
* To work as part of the multidisciplinary team under the direction and supervision of the registered staff, in a variety of community settings including patient's homes.
* To deliver indirect tasks which support the patient’s care and therapy needs, such as record keeping and management of equipment.
* The role includes weekend working.
* A full Driving Licence valid for the UK and access to a vehicle insured for business use is essential for independent travel across the work locality.

Person specification


Qualifications

* GCSE A‑D or grade 3‑9 in English and Maths or equivalent
* NVQ Level 2/3 in care/rehabilitation or equivalent
* Skills to manage challenging situations
* Good organisational skills/decision making


Experience

* Experience of working in healthcare or rehabilitation setting
* Experience working independently with people with physical and/or emotional needs arising from a long‑term health condition.
* Experience with admin duties
* Previous community experience
* Previous experience of being a generic worker within a multidisciplinary team
* Experience of working as a Therapy or Nursing Assistant/rehab technician
* Experience of working with people with impaired ability to communicate
* Experience working in the NHS


Skills, Abilities & Knowledge

* Full and valid driving licence
* Good communication and organisational skills
* Excellent IT skills, able to confidently use Microsoft Word, Excel, Outlook and Powerpoint
* Ability to deal with distressing and emotionally difficult situations
* Observation and reporting skills
* Understanding the needs of patients undergoing rehabilitation
* Ability to keep accurate, up to date records
* Knowledge of the role of other members of the multi‑disciplinary team
* Knowledge of the Mental Capacity Act (2005)
* Able to use SystmOne
* Experience of using electronic patient / service user record systems
* Able to evaluate data and produce reports using Microsoft Excel and Word


Communication

* Ability to engage well with people with an impaired ability to communicate
* Able to respond with compassion, sensitivity and empathy to people with emotional needs
* Able to communicate in English Language in both written and verbal modality
* Ability to communicate with patients with communication difficulties


Personal Attributes/Behaviours

* Able to drive a vehicle as this will be required as part of the role
* Able to identify with the Trust’s commitment to safeguarding and promoting the welfare of children and young people/vulnerable adults
* Evidence of demonstrating the Trust's values and behaviours
* Able to communicate effectively at different levels of the organisation and with staff, patient/service users, visitors or external organisations both verbally and in writing in the exchange of complex, sensitive or contentious information which may require the use of negotiating and/or persuasive skills
* Able to overcome barriers to understanding where there are physical or mental disabilities
* Able to analyse and assess situations and to interpret potentially conflicting situations and determine appropriate action, where there is a range of options and judgement is required
* Demonstrable ability of using tact and diplomacy
* Ability to work autonomously if required
* Willing to work flexibly and proactively with the team, including weekend and bank holiday working


Other

* Flexible and adaptable
* Able to work over a seven‑day rota
* Be able to kneel, bend & stoop, and work in cramped environments
* Able to manoeuvre limbs of around 5‑6 kg
* Able to manoeuvre patients using moving and handling aids
* Must hold full and valid driving licence
* Demonstrable skills in written and spoken English to a standard which enables the post holder to carry out the full range of duties and responsibilities of the role effectively
* Subject to the provisions of the Equality Act, able to travel using own vehicle on Trust business
