Overview

We are a multi agency team which provide health and wellbeing education and promotion to adults and older people in their own homes, over the telephone and via other routes i.e. group sessions and community events. We also help deliver the NHS Health Checks program in the community and at outreach venues across West Sussex. This is a great role for a community nurse with enthusiasm for health promotion and is a fixed term position for 9 months.


Responsibilities

* Complete holistic assessments of patients' health, social and environmental needs either by phone or during home visits.
* Deliver NHS Health Checks in community settings and outreach venues, supporting individuals in accessing health and wellbeing services through behaviour change.
* Collaborate closely with PAT colleagues, primary care teams, social services and the voluntary sector to ensure integrated service delivery and avoid duplication.
* Cover the Duty Line and support the maintenance of a service directory for accurate signposting.
* Target resources to populations with the greatest need, guided by Public Health data and the multi‑agency PAT Board.
* Occasionally supervise student nurses during community placements.
* Attend regular PAT & NHS Nurse Advisor team meetings, share lessons and support the development of new services.
* Develop, expand and implement a health‑enhancing, population‑based approach to improving the health and wellbeing of adults and older people in the community.
* Plan and implement activities designed to promote independence and wellbeing amongst adults and older people living in the community.
* Maintain up to date and contemporaneous records in line with National and Trust policies and procedures.
* Participate in clinical supervision and commit to continual professional development within the NMC Code and Sussex Community NHS Foundation Trust policies.
* Attend all statutory and mandatory training and any other training required for the role.


Essential Qualifications & Skills

* Registered nurse with the NMC.
* Relevant experience working with adults and older people in community settings.
* Post‑qualification experience and/or further education in health or social care related subjects.
* Knowledge of the boundaries of the role and of clinical governance.
* Understanding of statutory Health & Social Care Services (e.g. The Care Act 2014, National Service Frameworks for Older People).
* Knowledge of the NHS Health Checks Programme.
* Clear understanding of multidisciplinary working in the care of adults and older people.
* Excellent written and oral communication skills.
* Ability to provide and receive complex, sensitive information.
* Good knowledge of computing skills (e.g. Windows).
* Ability to work as an effective member of a team and independently, using initiative.
* Knowledge of NMC guidelines.


Desirable Qualifications & Skills

* Advanced computing skills (e.g. database skills, advanced spreadsheet skills, PowerPoint).
* Presentation skills.


Other Role Requirements

* Empathetic and non‑judgemental approach.
* Car driver with access to a car and a current driving licence, unless protected by the Equality Act.
* Flexible time management.
* Willing to learn new skills.
