Our Acute Stroke Unit has recently been relocated to a 26 bedded unit; the unit also has a TIA Clinic with dedicated Stroke Clinical Nurse Specialists and Stroke Support Workers. We have an established Clinical Nurse Specialist team who are based within the unit and work closely with the nursing team. The stroke team members will welcome you with open arms in a friendly and supportive environment, with a focus on providing excellent nursing and clinical care for stroke patients in their acute phase to achieve optimal patient outcomes. Excellent communication is vital for well-co-ordinated care across the Worcestershire Stroke Pathway, working with many specialist nursing teams, medics and therapists.

Responsibilities
* Acting as a Named Nurse, maintaining accountability for assessing, planning, implementing and evaluating programmes of care within the framework of team nursing
* Coordinate patient discharge arrangements in accordance with WAHT discharge policy
* Promote a patient-focused approach to care in collaboration with relevant health professionals
* Educate patients and their carers as required
* Provide information enabling patients to make healthier lifestyle choices; complete patient documentation correctly; assist in establishing and monitoring protocols/care pathways
* Support effective communication with patients, relatives and staff; uphold equality, diversity and rights; safeguard vulnerable adults per policies and procedures
Qualifications and experience
* Essential: Registered Nurse on sub part 1 or 2 of the professional register
* Essential: Evidence of continuing education for more experienced Band 5 nurses in the specialty
* Essential: Working towards stroke competencies of level 1 care within 4 months of starting, and level 2 care (HASU) within 11 months
* Desirable: Teaching and Assessing qualification
* Desirable: Significant knowledge and experience in the stroke specialty in an NHS setting
* Desirable: Ability to evidence learning and application in the workplace
