Neonatal Community Outreach Nurse

Northern Lincolnshire and Goole NHS Foundation Trust is seeking a dedicated Neonatal Community Outreach Nurse to provide community-based care for vulnerable neonates following discharge from the Neonatal Unit and Post‑Natal ward. The role involves home visits across North Lincolnshire, supporting families and coordinating with neonatal, maternity and community teams to ensure safe early discharge and ongoing health monitoring.
Responsibilities
* Provide community follow‑up to neonatal infants post‑discharge, including home visits, support with feeding, blood tests and home oxygen management.
* Facilitate support groups to maintain contact with infants and promote parental wellbeing.
* Work autonomously, accountable for own practice, under supervision of the Ward Manager and Paediatric Matron.
* Collaborate with staff on the Neonatal Unit and Maternity Wards to secure safe and timely early discharge.
* Ensure compliance with clinical protocols and maintain up‑to‑date records of all care interventions.
Person Specification
* Registered Nurse – Child or Adult Branch or registered Midwife.
* Qualification in speciality – Neonatology 405 or equivalent.
* Newborn Life Support course certification.
* Teaching and Assessing Qualification.
* BSc Hons in Community Children's Nursing.
* NVQ Assessor.
* Significant experience in clinical practice on a neonatal unit.
* Demonstrable high level clinical skills.
* Evidence of continuing professional development.
* Community Nursing experience.
* Evidence of developing education/training materials, presentations or packages.
* Evidence of research skills and implementing service development projects to completion.
* Shows desire and capability to develop in nursing care standards and follows evidence‑based practice.
* Can work on own initiative and collaborate effectively within a team.
* Demonstrates understanding of confidentiality.
* Plans, assesses, evaluates and implements patient care.
* Prioritises care in a changing environment or in response to patient demands.
* Facilitates learning of others.
* Exhibits high level of interpersonal skills.
* Demonstrates empathy and diplomacy.
* Flexible and reliable.
* Good time‑keeper.
* Able to work from both hospital sites as required.
* Full UK driving licence.
