An opportunity has arisen for a passionate and enthusiastic individual to join our innovative community service in Ely. We welcome applications from newly qualified Mental Health Nurses, Mental Health Occupational Therapists and Social Workers who are looking to move beyond their first qualified post to a community role.


Qualifications and Registration

* Relevant pre‑registration experience in mental health environments (not essential).
* Previous experience and transferable skills such as good communication, organisational ability and timekeeping.
* Applicants with lived‑experience of mental health challenges are especially encouraged.
* Passion for working with older people in the community experiencing mental health challenges.


Role and Primary Responsibilities

* Provide support interventions for people with significant mental health conditions.
* Work within a multi‑disciplinary team, collaborating with colleagues in the Mental Health Teams.
* Monitor and review the use of prescribed medications.


Additional Responsibilities and Skills

* Provide an ongoing assessment of care needs and plan care in collaboration with the service user, their carers (where appropriate) and MDT colleagues; deliver care safely and review needs in a timely manner.
* Support medication concordance in partnership with MDT colleagues, demonstrating knowledge of medications and policies used in older people’s mental health services, including relevant NICE guidance (e.g., antipsychotic prescribing in dementia).
o a. Clinical case management and supervision groups.
o b. Lead or participate in clinical meetings/reviews as appropriate.
o c. Attend administrative/management meetings as required.
* Actively engage in partnership working with patients, carers and service development.
* Maintain a working knowledge of current mental health and wider legislation, and effectively apply CPA assessment, care‑planning principles and risk assessment.
* Contribute to the assessment, treatment, evaluation and effectiveness of care offered, and review plans with the multidisciplinary team or team manager.
* Liaise effectively with other members of the service team, primary health care teams, residential/nursing homes, adult social care and other organisations relevant to the client.
* Employ specific skills to resolve/manage problems in older people’s mental health (e.g., anxiety management strategies, behavioural management strategies in dementia, family support skills).
* Familiarise yourself with and observe statutory and local policies in the prescribing, custody and administration of medications in line with the Trust’s medicine‑management policies and procedures.
* Maintain high standards in written reports, records of visits/interventions and care plans, recording client progress in accordance with record‑keeping standards.
* Keep up to date with nursing practices, ensuring PREP requirements are met, and stay informed about current trends and research findings in this specialised area of practice.
* Engage in clinical supervision and mentorship to enhance professional development.


Travel and Driving

In this role you will be required to travel independently around the county, meeting strict time deadlines. You must hold a full UK driving licence and have access to a vehicle. Public transport will not be used for this role.
