Job overview
A newly created vacancy within Mental Health Liaison Service in Pembrokeshire will enlarge and enhance the existing provision.
This specialty is concerned with the care of people presenting with both mental health and physical health symptoms regardless of cause. The service has a focus on the bio/psychosocial model, being concerned with the relationship between the physiology, psychology and sociology of human health and wellbeing.
We are looking for a motivated, experienced, and skilled Mental Health Nurse/Practitioner who is committed to helping improve and deliver the highest standards of person centred assessment and care to individuals presenting to the A&E department or within the general hospital experiencing mental health problems.
This post is primarily based in Withybush Hospital, with occasional inpatients in South Pembs Hospital or on Tenby Cottage Ward. It is a full time post, but part time hours would also be considered.
Main duties of the job
As an autonomous practitioner, you will demonstrate self-management skills and independence in thought and action and take personal responsibility for professional performance. The successful candidate will be part of a wider multi-disciplinary team, using a collaborative approach to solve and manage problems for the people in our care.
Therefore, an essential attribute needed for this post is the ability to work in collaboration with the multidisciplinary team and with voluntary organisations, local authority and other appropriate health and social care sectors to negotiate care provision across the primary/secondary care interface.
The successful applicants must have an interest working with adults aged over 18, possess excellent communication skills and be able to demonstrate a willingness to develop within the liaison role and as a member of the team. This includes being available to give advice at all stages of care. The aim is that assistance is delivered to patients in order to alleviate distress, prevent escalation and improve safety and patient experience.

The ability to speak Welsh is desirable for this post. English and/or Welsh speakers are equally welcome to apply.

Qualifications and Knowledge
* First level professional qualification in Mental Health /Learning Disability Nursing
* Excellent clinical knowledge of specialist area in particular comorbid mental and physical health presentations
* Evidence of post- registration continuing professional development activity to postgraduate diploma level
* Training in Brief Psychological Interventions
* Interest or experience in treating drug and alcohol misuse
Experience
* Evidence of achievement in clinical practice within an area of speciality, for instance inpatient, community, crisis team, drug and alcohol
* Experience of Liaison work
* Experience of working within a multi-agency community mental health setting
