Responsibilities:
* Assess the care needs of patients and develop a programme of evidence-based practice care which is evaluated and amended as appropriate without direct supervision.
* Maintain clinical competence through CPD and mandatory training requirements.
* Ensure clear and timely record keeping of all assessments and care provided.
* Maintain good communication with patients and their relatives including the MDT.
* Utilise, maintain and understand NUH evidence-based policies and guidelines to ensure safety of care.
