The post holder will be responsible for the assessment, diagnosis,treatment, referral, and discharge of a diverse range of patients with minorinjuries/minor illnesses, without routine reference to a doctor and withintheir level of competence and agreed protocols.

Advanced clinical practice is delivered by experienced, registeredhealth and care practitioners. It is a level of practice characterised by a high degree of autonomy and complex decision making. This is underpinned by a masters level award or equivalentthat encompasses the four pillars of clinical practice, leadership andmanagement, education, and research, with demonstration of corecapabilities and area specific clinical competence.
Advanced clinical practice embodies the ability to manage clinicalcare in partnership with individuals, families and carers. It includes theanalysis and synthesis of complex problems across a range of settings, enablinginnovative solutions to enhance peoples experience and improve outcomes.
This definition therefore requires that health and careprofessionals working at the level of advanced clinical practice will exerciseautonomy and decision making in a context of complexity, uncertainty andvarying levels of risk, holding accountability for decisions made.

Job responsibilities
To ensure Practitionerspossess the required knowledge, skills, and abilities to provide safe andeffective care, specific competencies for certain roles will need to becompleted before working autonomously in that field. For example, streaming,remote triage, home visiting and base.
KeyResponsibilities
ClinicalCare
Act as an advanced practitioner, demonstrating advanced clinicalcompetence and a knowledge
base beyond those associated with conventionalclinical roles.
Triage patients presenting as walk-ins to the appropriate service.
Undertake consultations, both face to face and via the telephone,with patients of all ages (neonate to elderly) as an autonomous practitionerand using own clinical judgement to diagnose, treat, refer and/or dischargepatients.
Make direct referrals to primary, secondary, and social serviceswithin locally agreed pathways, guidance, and protocols.
Provide clinical advice to operational and support staff whererequired.
Ensure that patient receive high quality clinical care, deliveredin a timely manner.
Prescribe, supply, and administer medicines as indicated toaddress patient need.
Ensure complete and accurate documentation of each patientcontact.
Work as an autonomous practitioner and as part ofmulti-disciplinary and multi-agency teams to ensure patients needs are met.
To undertake consultations via telephone.
