Job Summary
Join the Hospice at Home team as a Specialist Nurse/Practitioner, delivering compassionate care to patients in their homes. You will work closely with Clinical Nurse Specialists to support team development and help patients achieve their preferred place of care.
Responsibilities
* Provide high-quality palliative care to terminally ill patients in the community.
* Collaborate with the Clinical Nurse Specialists to develop and implement care strategies.
* Communicate effectively with patients, families, and multidisciplinary teams to understand and respond to care needs.
* Adapt to innovative ways of delivering care and support more patients at home.
Qualifications
* Relevant degree or diploma in nursing or healthcare.
* Demonstrated experience in palliative care or oncology.
* Exceptional communication and interpersonal skills.
* Motivated, adaptable, and able to work in a multidisciplinary environment.

Salary: £41,000 – £50,000 (depending on experience)
