Public Health Nurse Development Role

We deliver early intervention, health promotion and build therapeutic relationships to empower families, children and young people to make the best choices.

The Public Health Nurse role is an opportunity to work within the Healthy Child Service and then as opportunity arises onto applying for the Health Visitor/School Nursing training.

The Healthy Child Service prides itself on our supportive, skill mix team. You will receive regular 1-1s, safeguarding, caseload and management supervision, shared team learning and annual appraisals.

You will partake in regular whole team meetings where we value the voices of staff as paramount in the innovation of our service.

There are positions available in North Norfolk and Norwich City.

Main duties of the job

You will be working within the skill mix delivering the Healthy Child Service in a variety of settings, including home and community venues.

You will be working closely with partner agencies and voluntary services to ensure that the right support is given at the right time to children, young people and families.

You will be identifying need and delivering support in conjunction with the wider skill mix.

You will be working with children on the Universal to specialist pathways which includes safeguarding and be supporting the most vulnerable families.

You will be delivering evidence based practice in line with the NMC code, with an expectation to maintain knowledge and skills as a part of your ongoing professional development.

Job responsibilities

As part of the Healthy Child Service 0-19 team, the Public Health Nurse will be supported to work within a defined area, assessing need, delegating work within the team, implementing planned programmes of care and evaluating care delivered. The post holder will develop the skills to provide a child centred, inclusive, holistic and accessible Public Health Service for children, their families and communities.

Their role is to provide a child centred, inclusive, holistic, and accessible Public Health Service to children aged 0-19, their families and communities, establishing links with children, families, schools, communities and agencies (Education, School Services, Health and Voluntary organisations). Working on early intervention, health promotion, advocacy, empowerment and inclusion, to reduce health inequalities and ensure children are supported to reach their full potential.

Main Duties and Responsibilities

To work as part of a multidisciplinary 0-19 team to identify the health needs of children and young people, their families and the community and contribute to developing a strategy to address individual identified needs as well as those needs within the community.

To be committed to maintaining an evidence-based work ethic through continual learning and professional development. Post holder will be encouraged to develop professionally including consideration of the SCPHN course.

To develop further their core skills of assessing, planning, implementing and evaluating care to provide early intervention and care to families, children and young people in line with our commissioned service.

To support families through a range of methods including home visits, digital or in person drop‑in clinics, virtual and phone contacts to assess the needs of children and their families.

To work as part of the locality team to support children, young people and families who are vulnerable or at risk. Post holders should ensure that they are compliant with safeguarding mandatory training and that they are aware of local safeguarding policies and procedures and how to access these.

To support other localities as required, including Just One Number, to ensure that all relevant clinical skills are developed, and to maintain effective working relationships and continuity of services.

To work with, promote and support families to access the Norfolk Family Hub offer.

To develop skills to promote the use of digital resources on Just One Norfolk during all contacts with service users to enhance self‑care skills for children, young people, and their families.

To take responsibility for their own clinical practice including supervision and to adhere to the NMC code.

To contribute to the development of local policies and procedures relevant to their area of work and contribute to relevant service design projects.

To develop the skills to utilise virtual platforms in assessments, interventions, training, and networking to support effective practice and partnership working.

Within this role you will need to travel as an independent practitioner on a daily basis, across Norfolk which involves both rural and urban travel. The nature of our work does result in travel arrangements changing at short notice. Due to the large geography our service covers and the limitations of sufficient public transport to undertake this role you must hold a full and current driving licence and have access to your own transport. This is subject to the provisions of the Equality Act 2010, and therefore if the post holder requires a reasonable adjustment due to a disability then this will be considered.

Post holders will be willing to undertake training to be an assessor and supervisor to enable them to support pre‑registration students on placement within the service. Post holder will ensure that they maintain compliance with this requirement.

Collaboratively facilitate in delivering our Pathway to Parenting (P2P) programme to expectant parents in the community.


Person Specification


Qualifications

* Current NMC Registration
* Evidence of degree level study in the last 2 years (or willingness to work towards)
* Mentor/998 or equivalent teaching qualification


Experience

* Experience of working in a multidisciplinary setting
* Experience of Safeguarding adults and/or children
* Experience of Mentoring
* Experience of working in a community setting
* Experience of working with children and their famillies


Knowledge and Skills

* The clinical skills to identify, respond to and evaluate health needs
* Current knowledge of local and national policies informing the delivery of the Healthy Child Programme
* Competent keyboard skills and ability to communicate through IT using packages such as Word, Outlook, Microsoft Teams, Excel and PowerPoint
* Commitment to the development of excellent services, which focus on improved outcomes for children
* Demonstrate an understanding of and ability to act upon the principles and practice of health promotion
* Able to plan and organise their work using time management and prioritisation skills and work independently
* Ability to communicate effectively with children, parents and partner agencies
* Ability to work flexibly as part of a team
* Experience with delivering healthcare interventions via digital platforms
* Ability to use SystmOne
