Post Title: Family Support Leader ​ Reporting To : Senior Family Support Leader ​ Location : The post will be based at Daisy Chain but will be required to travel across the Sunderland locality on a regular basis ​ Hours: 30 hours per week (Job Share can be considered) ​ Salary : £26,000 per annum, pro rata ​ Duration of Contract: Fixed term until 30th June 2027 JOB SUMMARY Supported by the Senior Family Support Team Leader the post hold will work within a small team to provide bespoke support, guidance, training and strategies to families whose child has a neurodevelopmental difference, whether diagnosed or not. Please note this post has come about due to a new contract to provide a Family Support Service within the Sunderland locality. It is funded by the ICB. The post holder must be willing to travel and work flexibly to meet the needs of the families who will access the service. ​ KEY TASKS ​ Act as the main point of contact for families in need of support and signpost to other services and teams as appropriate ​ Asses the needs of families that are new to the service in order to be able to signpost them to the most appropriate support and services. ​ Provide a bespoke package of support that is tailored to the needs of the individual families and may include providing strategies and advice, written information and liaising with other services to ensure they get the support that they need. ​ Train as a Sleep Counsellor and deliver a package of sleep support and learning workshops to parents/carers who have a child/young person with an identified additional need. ​ Build and maintain positive, trust based relationships with families and young people/adults, in order to identify the key issues which are relevant to them. ​ Plan and deliver 1:1 support and group support activities such as parent coffee mornings and drop-in sessions. ​ Maintain and develop an up to date knowledge of local service provision for appropriate signposting and referrals. ​ Contribute towards the protection of individuals from abuse by adhering to Daisy Chain’s safeguarding policies at all times. ​ Ensure confidentiality is maintained and that Daisy Chain’s GDPR Policy is adhered to at all times. ​ Administration: Ensure effective and accurate recording and reporting systems in line with Daisy Chain’s procedures. ​ Undertake administrative tasks necessary for the efficient and effective delivery of the key responsibilities in this role. ​ Additional responsibilities: Comply with the Charities’ Health and Safety policy in all employment practices ​ Maintain the integrity of the Daisy Chain brand in all internal and external communications. ​ Work in ways, which promote equality of opportunity, participation and responsibility. ​ Maintain a proactive approach to your role and responsibilities along with a commitment to contribute and maintain the positive working ethos within the Charity. ​ Undertake any training and development deemed necessary for the pursuance of the post. ​ Ability to travel within then community to provide planned services. ​ Any other duties from time to time, commensurate with the level of the post ​ This job outline is intended to indicate the broad range of responsibilities and requirements of the post. It is neither exhaustive nor exclusive but, while some variation can be expected in particular duties, the outline is considered to provide a reasonable general description of the post. ​ This post is deemed to require an Enhanced DBS Check with the Disclosure and Barring Service due to the fact that there is access to children and vulnerable adults. ​ Person Specification ​ Experience: ​ Experience of delivering support services to families, young people and adults with complex needs including autism – E, A&I Experience of multi-agency working – E, A&I Experience of working with individuals with Autism Spectrum Disorders and other Neurodevelopmental Disorders – E, A&I Experience of working in a public health project – D, A Experience of delivering training to families– D, A&I Experience of safeguarding vulnerable children and young peoples – E, A&T Qualifications: Level 3 qualification in a relevant field (e.g. autism, education, health & social care) – E, A Trained to level of Designated Safeguarding Officer (or willing to work towards) – D, A Knowledge and Skills: Ability to communicate effectively in sensitive and complex cases with individuals from a variety of backgrounds and with varying needs. – E, A&I An ability to remain calm, objective and proactive when faced with complex cases, including safeguarding – E, A&I A thorough understanding of the challenges faced by individuals with neurodevelopmental differences – E, I Excellent written and verbal communications skills – E, A/T/I Excellent listening skills and the ability to be sympathetic and discreet – E, I IT literate and able to use all appropriate software packages – E, A Excellent record keeping skills – E, I Other: Understanding of and commitment to the aims, ethos and philosophy of Daisy Chain – E, I A commitment to maintaining confidentiality and a clear DBS – E, A Ability to travel to projects, events and meetings – E,A Clean full driving license with own transport – E, A Ability to work well independently and as part of a team – E, A
