Enfield Unity Primary Care Network is the second largest PCN in England representing over 158,000 patients across 17 GP Practice sites. Our aim is to improve services and outcomes for patients by supplying a well-trained, motivated workforce.

An exciting opportunity has arisen for a part-time social prescriber to join the personalised care team consisting of health coaches, outreach workers, care co-ordinators, social prescribers and health and wellbeing coaches.


Main duties of the job

This is a non-clinical role, that supports patients to address and tackle issues that are having an adverse effect on their mental and physical health such as housing, debt or social isolation.

Your role within this team will include taking referrals from PCN practices, assessing individual patients and managing a patient caseload, providing signposting to different services and connections with community groups and services. These services may include befriending, weight management, mental health support, smoking cessation, local social groups, leisure centres and volunteering, forging working relationships and creating referral pathways with VCSE organisations and many more.

As a Social Prescriber link-worker in Enfield Unity PCN, you will join a dynamic multi disciplanary team, helping to make a real difference to peoples lives by empowering them to take control of their health and wellbeing, using a holistic approach and connecting people to community groups and statutory services for practical and emotional support.


About us

Medicus Health Partners (MHP) is the lead practice for Unity PCN and will be the employing organisation.

The successful applicant will have access to the training, support, and governance systems in place at MHP.

Applications are welcome from those with existing social prescribing/ social care experience.


Job responsibilities

Referrals & Assessments

* Receive referrals from GPs, nurses, and other healthcare professionals.
* Conduct holistic assessments to understand patients needs, strengths, and goals.
* Prioritise referrals based on urgency, risk, or complexity.
* Develop personalised care and support plans with individuals.
* Motivate and support clients to take active steps toward improving their well-being.

Connecting to Community Services

* Signpost or refer individuals to a wide range of services such as:
* Housing support
* Mental health services
* Social clubs and volunteering
* Physical activity and lifestyle groups
* Liaise with voluntary, community, faith, and social enterprise (VCFSE) organisations.

Follow-up and Monitoring

* Monitor progress and offer ongoing support where needed.
* Maintain regular contact and review goals.
* Use case management systems to record interactions, outcomes, and feedback.

Collaboration and Communication

* Build strong relationships with healthcare teams, local authorities, and community groups.
* Attend multidisciplinary team meetings and contribute to patient care plans.
* Promote awareness and understanding of social prescribing among colleagues.


Person Specification


Qualifications

* Higher level of education (e.g. NVQ level 3 or A levels at minimum or equivalent)
* Completion of E-learning for health or other accredited program for Social Prescribers
* Demonstrate commitment to personal and professional development
* Experience with EMIS clinical systems
* Previous experience working in primary care, the NHS, or Social Care
* Experience working with people with learning disabilities and/or serious mental illness
