Job Title: Social Worker – Adult Community Team
Location: Liverpool
Rate: £43.78 per hour
An exciting job role has recently become available In Liverpool. They are looking for a new member for their Adult Community Team.
What will your responsibilities be?
Your responsibilities will include case management, where you will assess the needs of adults and develop personalized care plans to help them achieve their goals and maintain their independence. You will provide immediate support during crises, helping individuals navigate significant changes or challenges in their lives. Advocacy is a key part of your role, ensuring clients have access to necessary services and resources. You will engage with community organizations to develop programs and services that benefit the adult population. Additionally, you will educate the community about issues affecting adults, such as elder abuse, mental health, and available resources. Regularly reviewing and adjusting care plans to meet patients’ evolving needs, maintaining accurate records of assessments and interventions, and collaborating with multidisciplinary teams to provide holistic care are also essential aspects of this role. Key skills for this role include strong communication, empathy, and the ability to work under pressure.

Qualifications and Experiences:
To be successful in this role, you must have Social Work England registration, be eligible to work in the UK, hold a qualification in Social Work at degree level, and have post-qualified experience.
