Job Overview
An exciting opportunity has arisen for a Band 4 nursing associate to work within the Mid Cornwall Locality in the Older People's Mental Health Community Service. This is a newly developed role and the successful applicant will work within the Multidisciplinary team, completing mental health reviews for patients and supporting the Care Co‑ordinators in their role.
The role is for 37.5 hours per week, working Monday to Friday 09:00 – 17:00 (excluding a 0.5 hour unpaid lunch break each day). Successful applicants must be able to travel independently to patients’ houses within the Mid Cornwall area and will require loan working.
Responsibilities
* Complete mental health reviews for patients presenting with acute and severe behavioural and psychological symptoms of dementia of any age, and functional mental health disorders over the age of 75.
* Hold a caseload of patients requiring mental health support, including medication tolerance reviews, risk reviews and care plan reviews.
* Conduct reviews of specific conditions, covering both functional and organic mental health disorders.
* Administer long‑acting injectable antipsychotic medications and monitor for side effects.
* Conduct reviews for patients under Section 117 of the MHA (1983).
* Support the Community Psychiatric Nurses within the service in their roles as Care Co‑ordinators and Duty Clinicians, working within the multidisciplinary team.
