The successful candidate will support patients across the PCN to providehealth and wellbeing support interventions. HOSMS PCN will provide supportwhich will include supervision and mentorship. Experience in general practiceor healthcare settings would be an advantage but is not essential. Moreimportant for this exciting role is a positive attitude and expertise inengaging and motivating people, using health coaching techniques to supportthem towards behavior change.

About us
HOSMS Primary Care Network is located in East Devon and has a population of around 50,000 patients. There are 3 surgeries within the PCN, with excellent inter-personal relations, a good support network and a history of collaborative working. The practices are Honiton Surgery, Coleridge Surgery and Sid Valley Practice.
The PCN has a number of teams which work across the three practices, including the Pharmacy Team, Health and Wellbeing Team and FCP Physiotherapy Team. The teams are supported by a Lead Manager, Deputy PCN Manager, Clinical Director as well as clinical supervision from other members of the three practices.

Job responsibilities
HOSMS Primary Care Network have an exciting opportunity for a Health andWellbeing Coach to join their successful PCN Health and Wellbeing Team. Thisestablished team has been running health and wellbeing services includingHealth and Wellbeing Coaching, Social Prescribing, Group Work, Signposting andStaff Wellbeing Support for over five years across Honiton Surgery, ColeridgeMedical Centre & Sid Valley Practice.
JOBRESPONSIBILITIES:
Triage and assess patients under the guidance of thePCN Clinical Lead
Manage and prioritise a patient list
Provide 1:1 health coaching interventions both inpractice and virtually for patients identified as having mild to moderatemental health needs, physical health needs or requiring support with behaviourchange to improve their health.
Use a non-judgemental, strengths and recovery basedapproach which considers the person as a whole when addressing their healthissues
Use a number of tools to empower patients to takecontrol of their own health
Provide interventions such as coping skills,self-management and education relevant to the individual
Coach and motivate patients through multiple sessionsto identify their needs, set goals and support them to achieve theirpersonalised health and care plan objectives.
Facilitate group health coaching interventions such asHOPE Programme
Encourage patients to actively participate in theirrecovery journey using a working knowledge of community assets, increasingtheir ability to access and utilise community support offers
