Job Summary We are currently seeking a P/T GP 2/3 days per week to join Horsman’s Place Surgery. Main duties
* Provide primary medical services to the registered patients of the practice, both telephone and face-to-face (F2F), ensuring high standards of care in line with national primary care contractual regulations and guidelines.
* Deliver care for on-the-day and pre-booked patients with long-term conditions, plus some administrative tasks.
About Us
We are a friendly, supportive training Practice in Dartford, Kent with approximately 9,500 patients. Services include asthma/COPD and diabetes clinics, childhood immunisations, travel advice, and minor surgery. Our multidisciplinary team includes GPs, Nurses, Physiotherapists, and a Pharmacy Team. Phlebotomy is also offered. We welcome and support special interests.

Job Description & Responsibilities
All normal duties and responsibilities associated with a Salaried GP working within primary care as set by regulatory and standard-setting organisations. Duties include:
* Surgery consultations, telephone consultations, and E Consults; triaging, checking and authorising repeat prescriptions; handling patient queries, test results, medical reports, and examinations on behalf of the practice.
* Provide health promotion and advice; signpost patients to appropriate services to maximise health and social functioning and their ability to self-manage care.
* Maintain personalised care and population health, including reducing health inequalities, reflected in daily practice.
* Make professional, autonomous decisions for presenting patients, whether self-referred or referred from other health care workers or external organisations.
* Assess health needs of patients with undifferentiated/undiagnosed problems; take appropriate action.
* Screen patients for disease risk factors and early signs of illness; take appropriate action.
* Record clear, contemporaneous consultation notes using IT systems.
* Prescribe in line with locally or nationally agreed formulary.
* Work according to relevant clinical guidelines, pathways and protocols.
* Be responsible for tasks delegated appropriate to the level of in-hours duty doctor sessions; to be agreed.
* Participate in continuing professional development, annual GP appraisal, revalidation and peer supervision; stay up to date with professional developments. Utilise study leave to support development needs identified in PDP and appraisal. Undertake requirements to remain on the Medical Performers List.
* Stay up to date with good clinical practice guidance (e.g. NICE) and implement relevant guidance.
* Engage with professional development lead to promote patient safety and quality of service.
* Participate in clinical governance activities, including clinical audits and quality improvement activities as required by external accrediting bodies.
* Ensure that Care Quality Commission (CQC) requirements are reflected in day-to-day practice and maintain management of CQC requirements.
