Personal Advisor (Leaving Care Service)
Purpose of the Role
Provide practical, emotional, and advocacy support to care-experienced young people transitioning into adulthood, helping them achieve positive outcomes in education, employment, housing, health, and independent living. Build trusted relationships with young people, ensuring their voices, wishes, and aspirations are central to planning and decision-making while working in partnership with multi‑agency professionals to promote safety, wellbeing, and successful transitions from care.
Key Responsibilities
* Support care leavers through regular direct work, visits, assessments, and ongoing engagement.
* Develop and maintain strong, trusting relationships with young people, using trauma‑informed and person‑centred approaches.
* Complete Pathway Plan assessments, reviews, and visits in accordance with statutory guidance and departmental procedures.
* Advocate for young people and ensure their views, wishes, and feelings are represented in all planning and decision‑making processes.
* Coordinate and work collaboratively with housing, education, employment, training, health, and community services to secure positive outcomes.
* Provide guidance and practical support around accommodation, independent living, budgeting, benefits, education, training, and employment opportunities.
* Identify safeguarding concerns and implement child protection procedures where children or young people may be at risk.
* Promote awareness of safe care, exploitation risks, and personal safety.
* Encourage participation in community‑based activities that support social inclusion, confidence, and personal development.
* Maintain accurate case records, reports, assessments, and documentation in line with organisational policies and statutory requirements.
* Participate in duty systems, supervision, team meetings, training, and service development activities.
* Organise and facilitate group work sessions and support young people to engage with support networks and community resources.
Key Skills and Competencies
* Strong relationship‑building and communication skills with young people, families, and professionals.
* Ability to conduct needs assessments and develop outcome‑focused support plans.
* Knowledge of trauma‑informed practice, safeguarding, child development, and attachment theory.
* Effective case management, organisation, prioritisation, and record‑keeping skills.
* Experience of multi‑agency working and advocacy.
* Empathetic, non‑judgmental, and resilient approach when supporting vulnerable young people.
* Commitment to anti‑discriminatory, inclusive, and strengths‑based practice.
* Ability to motivate and empower young people to achieve their goals and aspirations.
* Competent IT and administrative skills, including maintaining accurate electronic records.
