We are looking for an enthusiastic, motivated Nurse to join our forward‑thinking practice.
Salary is competitive, depending on experience, with access to NHS pension scheme.
The position is part‑time for a minimum of 16 hours per week, but more may be offered to the right candidate. Training will be given to integrate you into our team.
Job responsibilities
* Act as the Infection Prevention Control (IPC) lead, work to the IPC Policy, provide audits, training, support and evidence of compliance and the annual statement.
* Develop, implement and embed health promotion by participating in national screening and well‑being programmes including ECG monitoring, blood pressure, cervical cytology, etc.
* Identify, manage and support patients at risk of developing long‑term conditions, preventing adverse effects to patient health.
* Provide routine nursing care to patients as required in accordance with clinical evidence, NICE and the NSF including ear syringing, venepuncture.
* Provide wound care (ulcer/Doppler etc.) to patients.
* Request pathology services as necessary.
* Provide chronic disease clinics, deliver patient care as necessary, refer patients to secondary/specialist care as required.
* Maintain accurate clinical records in conjunction with extant legislation.
* Ensure Read codes are used effectively.
* Maintain chronic disease registers.
* Chaperone patients where necessary.
* Assist GPs with minor surgery when required.
* Prioritise health issues and intervene appropriately, give advice on health, lifestyle, diet, smoking, alcohol intake, sexual health and contraception.
* Support the team in dealing with clinical emergencies.
* Recognise, assess and refer patients presenting with mental health needs.
* Implement vaccination programmes for adults and children.
* Support patients in the use of their prescribed medicines or over‑the‑counter medicines (within own scope of practice).
* Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients, etc.).
* Delegate clinical responsibilities appropriately, ensuring safe practice and that tasks are within the scope of practice of the individual.
* Support the clinical team with all safeguarding matters in accordance with local and national policies.
* Understand practice and local policies for substance abuse and addictive behaviour, refer patients appropriately.
* Deliver opportunistic health promotion where appropriate.
* Collect data and participate in regular practice audit and research projects.
* Contribute to the preparation of any practice development plans.
* Maintain own annual registration with the NMC and adhere to the Code of Professional Conduct.
* Maintain a safe and therapeutic environment for all staff, patients, carers and co‑workers from other agencies; ensure all statutory regulations (e.g. Health & Safety, COSHH, etc.) are implemented and adhered to.
* Maintain own continuous professional development and keep PDP up‑to‑date in line with the requirements of the practice.
* Attend and participate in staff and practice meetings and in‑house clinical training sessions where necessary.
Qualifications
* RGN qualification.
* Current NMC registration.
* Experience of working in a primary care environment.
* Experience of working as a practice nurse or community nurse.
* Experience of chronic disease management.
* Experience of infection prevention and control measures.
