FEP Social Worker/Care Coordinator

You will play a pivotal role in providing comprehensive and person-centred care to individuals experiencing early psychosis. You will work closely with a multidisciplinary team to deliver high-quality interventions that promote hope, recovery, resilience, and well‑being. Your responsibilities will include conducting assessments, developing care plans, and coordinating services to ensure that each individual receives the support they need to achieve their goals.


Main duties of the job

Job responsibilities

Key Responsibilities:

* Conducting thorough assessments to understand the needs and strengths of individuals experiencing early psychosis.
* Developing and implementing personalised care plans in collaboration with service users, their families, and other professionals.
* Providing ongoing support and monitoring progress to ensure that care plans are effective and responsive to changing needs.
* Facilitating access to a range of services, including mental health, social care, and community resources.
* Advocating for service users’ rights and ensuring that their voices are heard in all aspects of their care.
* Participating in regular team meetings, case reviews, and professional development opportunities.


Person Specification


QUALIFICATIONS/EDUCATION/TRAINING

* Academic qualifications at degree level or above
* Membership of professional bodies
* Additional areas of training through short courses


PREVIOUS EXPERIENCE

* Experience at in a relevant mental health setting
* Evidence of continuing professional development
* Working knowledge of MHA (1983), CPA, Risk Assessment and Section 25 etc.
* Effective multi-professional team communication & team working
* Ability to assess, plan, implement and evaluate agreed care with supervision


COMMUNICATION SKILLS

* Excellent ability to communicate with a wide range of stakeholders including service users and carers, other professional colleagues, members of other organisations and local communities, verbally and in writing
* Ability to manage highly sensitive information
* Experience of using electronic recording systems and ability to produce accurate, appropriate and timely reports


ANALYTICAL SKILLS

* Ability to analyse service user and carer information
* Experience of undertaking small scale audits and surveys


PHYSICAL SKILLS

* Ability to visit and attend meetings at a variety of locations and environments
* Experience of using computer – PC literate to ECDL standard


PHYSICAL EFFORT

* Office based and community based visiting service users in their own homes, travel within specified geographical area
* Ability to meet timescales and respond to crises as appropriate


MENTAL EFFORT

* Flexibility and ability to respond to a variety of tasks, and to prioritise own workload


EMOTIONAL EFFORT

* Ability to deal with distressing and sometimes aggressive behaviour


GENERAL

* Experience of working in an integrated health, psychological and social care environment
