Community Nursing Team Coordinator (Part-Time)

An exciting opportunity has arisen to join the forward-thinking and dynamic PCN community team in Crowborough, Ashdown Forest, and Wadhurst catchment area.
This pivotal role will provide prompt, efficient co-ordination and administration support to PCN teams, playing a significant part in the day-to-day administration of Community Nursing services.

Main duties of the job
* Provide prompt, efficient co-ordination and administrative support to community nursing services.
* Act as the main point of contact for service users, their families, and other teams and organizations.
* Respond promptly to incoming referrals via various sources, including SystmOne, email, and telephone.
* Communicate effectively with service users with complex needs using various methods and adapt communication accordingly.
* Coordinate interventions by prioritising and allocating Care Plans to appropriate team members, referring to Standard Operating Procedures, and escalating to the Senior Nurse on Duty when necessary.
* Support the clinical team to deliver high standards of care by ensuring continuity and effective coordination of services.

Person Specification
Qualifications
* GCSE passes at Grade C or above or equivalent experience
* Evidence of continuous learning through education, training, and experience
* Understanding of basic human needs: physical, emotional, health, and social
* NVQ Level 3 or equivalent in health and social care or business administration
* Knowledge of assessments & Care Management processes
* Knowledge of medical terminology
* Understanding of social care & health systems, processes, and pathways
Experience
* Significant experience working in a health or social care setting
* Previous administrative experience
* Experience working with multidisciplinary teams
* Experience working with vulnerable populations
* Extensive experience in public interaction
Skills & Knowledge
* Proficiency with recognised IT packages such as Microsoft Office, Excel, and Access
* Effective keyboard and data entry skills
* Ability to undertake statistical and data analysis
* Strong interpersonal skills with empathetic and tactful communication
* Excellent communication skills for engaging with customers, careers, and colleagues
* Ability to manage conflicting deadlines, prioritise tasks, and work independently
