As a Family and Carer Ambassador, you will provide practical and emotional support to families and carers of those with complex mental health needs. You will also offer advocacy to ensure that the families, carers, and patients' needs and views are heard and acted upon, regardless of their communication needs. You will identify and address cross-organizational challenges and work closely with key agencies across health, education, social care, and the voluntary sector, including links with adult services for transition planning or support.
Position available is located at Derwent Centre.
To succeed in this role, you must be proactive, compassionate, detail-oriented, well-organized, and possess strong communication skills. You should be passionate about effecting change and empowering patients, families, and carers. This is a new post with scope to shape and influence the role and develop relationships with stakeholders and partners.

3. Supporting communication between clinical units and families.

13. Providing carer support, including follow-ups and information dissemination.
14. Inviting carers to reviews and leading carers' groups.
15. Maintaining boundaries, documenting input, and referring to support services.
16. Building links with local charities and attending meetings like the Triangle of Care.
17. Creating relapse prevention plans and monitoring strategy progress.
Promoting Rights and Signposting:
1. Sharing information about care planning and legal rights.
2. Understanding community services and signposting to local groups and policies.
