Community Support Worker – DART
Location: Royal Sussex County Hospital, Eastern Road, Brighton, BN2 5BE.
Salary: £29,064 – £31,022 per annum (will rise from 1 April 2026). Hours: 37 hours per week, basis fixed hours.
Job type: Adult Social Care.
Role Summary: As part of the Discharge, Assessment and Review Team (DART) at Royal Sussex County Hospital, you will support patients’ safe return home by securing and protecting their household property, arranging cleaning or safety works, and ensuring pets are cared for. You will work independently in the community and collaborate closely with Social Workers, ward multi‑agency teams, housing providers, voluntary organisations and specialist services.
Responsibilities
* Visit and assess properties to ensure they are safe, secure and suitable for a return home.
* Coordinate cleans, decluttering, home‑safety interventions and key collection/return.
* Liaise with families, ward staff, housing providers, cleaning companies, animal welfare services and wider partners.
* Support arrangements for pets when patients are admitted to hospital, including liaising with kennels or emergency pet care.
* Record accurate information on Eclipse and maintain clear communication across the hospital system.
* Work independently in the community, make risk‑based decisions and escalated issues appropriately.
Qualifications, Experience & Skills
* Strong communication skills and ability to manage sensitive conversations with calmness and respect.
* Confidence in lone working and navigating varied home environments safely.
* Practical problem‑solving skills, particularly in coordinating multiple agencies and arranging property‑related tasks.
* Experience in Social Care, Housing, Health, Community Support or similar public‑facing work.
* Ability to prioritise, make decisions under pressure, and keep clear, accurate records.
