Job Summary
LOCALA CHILDREN'S COMMUNITY NURSING TEAM – Children's Community Nurse
Band 5 fixed‑term maternity leave cover (12 months), up to 30 hrs per week (negotiable if fewer hours required). Salary: £32,073 – £39,043.
Primary Responsibilities
Deliver high‑quality, child‑ and family‑centred care to children and young people with acute, chronic, and complex health needs across home, school, and clinic settings.
* Deliver evidence‑based nursing care to children and young people within the community.
* Support children with complex and additional healthcare needs; provide education and support to families and carers to promote independence and wellbeing.
* Work collaboratively with multidisciplinary teams, schools, social care, and partner agencies.
* Participate in assessment, care planning, safeguarding, and clinical reviews.
* Maintain accurate and timely clinical documentation in accordance with NMC standards.
* Contribute positively to service development and team learning.
* Provide nursing cover one day a week at a Special Educational Needs school alongside experienced colleagues.
