Shah Zaman Surgery is seeking an experienced and organised Care Coordinator to join our team. This part-time role offers 20hours per week, spread across 3 days, with a salary dependent on experience.
As the lead for all patient recalls, you will play a central role in ensuring patients receive timely, coordinated care, supporting delivery against QOF and the Universal Offer. We are looking for someone with experience in primary care, including managing chronic disease recalls, who is proactive, detail-focused, and committed to improving patient outcomes.
You will also contribute to safeguarding, Gold Standards Framework (GSF) work, provide administrative support to the Practice Manager, and may assist reception with tasks or patient calls when needed, as well as carry out other duties depending on the needs of the practice.

Main duties of the job
Take full responsibility for all patient recall systems.
Manage and monitor recall lists in line with QOF and Universal Offer targets.
Proactively contact patients, schedule appointments, and ensure follow-up.
Work with clinicians to ensure reviews, including chronic disease checks, are completed on time.
Safeguarding
Support safeguarding processes in collaboration with the Safeguarding Lead.
Organise safeguarding meetings, prepare agendas, and take accurate minutes.
Maintain and update the safeguarding register.
Escalate concerns in line with policy and statutory guidance.
Gold Standards Framework (GSF)
Coordinate GSF meetings and maintain accurate records.
Update and manage GSF registers.
Support communication between clinicians, patients, and families as required.
Practice & Administrative Support
Provide administrative support to the Practice Manager.
Assist with data quality, audits, and reporting linked to QOF and the Universal Offer.
Support service improvement projects as directed.
Act as a key contact for patients regarding recalls and care coordination.
Help patients navigate health and social care services.
Ensure patients understand care plans and have access to self-care information.
Communicate clearly, respectfully, and inclusively with patients and carers.

Job responsibilities
This is a key non-clinical role within Shah Zaman Surgery, responsible for leading all patient recall processes. You will ensure patients are invited and supported to attend their reviews in line with QOF and the Universal Offer, with a focus on chronic disease recalls, directly contributing to improved patient outcomes and the quality of care.
In addition, you will support the practices safeguarding responsibilities, contribute to GSF processes, and provide assistance to the Practice Manager. The role requires excellent organisational skills, strong attention to detail, and effective communication, alongside experience working in primary care.

Experience
* Experience working in a GP surgery or primary care setting
* Understanding of QOF and the Universal Offer
* Experience managing patient recall processes, including chronic disease recalls
* Excellent organisational and administrative skills
* Strong written and verbal communication skills
* Ability to prioritise, multitask, and work independently
* Awareness of and commitment to safeguarding principles
* Commitment to patient confidentiality and data protection
* Previous experience as a Care Coordinator or similar role
* Experience supporting safeguarding or GSF processes
* Knowledge of NHS primary care structures and PCN DES
* Experience contributing to audits and service improvement
