An exciting opportunity to support the use of an innovative AI tool across Mersey and West Lancashire Teaching Hospitals has become available. The tool helps prioritize patients for surgery through a self-care coaching prehabilitation programme. The Surgical Navigator role will focus on promoting and supporting the onboarding of eligible patients to wait well for surgery at home with the coaching programme. We are seeking an enthusiastic individual to join our Surgical Team to communicate the benefits of enrolling in the 12-week prehabilitation programme and guide them through the process. The role also involves liaising with surgical and pre-operative assessment colleagues to track updates for this patient group. The Surgical Navigator will generate and manage patient lists, liaise with clinical teams to capture review outcomes, and ensure data is accurately recorded for divisional reports.
Responsibilities include:
1. Planning and organizing activities to ensure weekly patient lists are prepared and available to clinicians for review.
2. Attending weekly operational meetings.
3. Developing procedures within the departmental guidelines.
4. Managing complex patient data using Excel and dashboards.
5. Ensuring patient lists are shared with third-party prehabilitation providers.
6. Identifying patient cohorts requiring harms review and exporting data for divisional use.
7. Managing review timelines and escalating delays.
8. Recording review outcomes accurately in digital patient notes.
9. Ensuring the quality and accuracy of data reports.
