Overview

Senior Therapy Support worker role involving assessing and treating patients in a range of daily activities to encourage functional independence in preparation for discharge from hospital. You will work under the guidance of an established and supportive team of qualified Occupational Therapists, Physiotherapists and other therapy support staff within the service. The OT and PT team practice shared skills and you will be supported to achieve selective competencies within physiotherapy to enhance the service for our patients. You will be supported to develop the skills to work independently, to provide delivery of care including liaison with patients, family members, carers, the multi-disciplinary team and external agencies following a “home first” model. IT skills are required as the service operates with electronic patient records. There is a commitment to support 7-day services, so flexibility to work weekends on a rota system will be required.


Responsibilities

* Assess and treat patients in a range of daily activities to encourage functional independence in preparation for discharge.
* Work under the guidance of an established team of Occupational Therapists, Physiotherapists and other therapy support staff; develop competencies within physiotherapy to enhance patient care.
* Support to develop the skills to work independently and deliver care, including liaison with patients, family members, carers, the multidisciplinary team and external agencies, following a "home first" model.
* Maintain and utilise electronic patient records; contribute to a patient-centred care approach and effective discharge planning.
* Commitment to 7-day services; flexibility to work weekends on a rota system.


Qualifications and Skills

* Experience working in a therapy environment with understanding of the physical, cognitive and emotional needs of patients experiencing an acute admission and the effect on discharge.
* Excellent written and verbal communication, flexibility and initiative; ability to prioritise workload in a busy environment.
* Excellent interpersonal skills and aptitude for team working; able to demonstrate the Trust’s PROUD to CARE values.
* Flexibility to help cover other wards as required by service needs.

* The post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975; submission for Disclosure to the Disclosure and Barring Service (DBS) is required.
