Overview

Advanced Clinical Practitioner – General Practice Visiting Service. We are seeking a highly motivated ACP to join our multidisciplinary team supporting our care home, acute, and home visiting services. We offer opportunities for personal development with annual appraisal, training and education. Work as an effective and responsible team member, exploring new ways of working to benefit the practices and the patients. This role works within the Enhanced Health in Care Homes (EHCH) model to support people living in care homes, providing a similar level of support as if they were at home. The post includes supporting housebound patients to improve management of long-term conditions (LTCs) and help reduce admissions. There is triage and rotation of attending acute home visits with the clinical team. Our aim is to provide high-quality, accessible, patient-focused and sustainable healthcare.


Main duties of the job

* Work with existing clinicians to support GPs in the practice with care-home visits and ward rounds, housebound patients and acute home visiting.
* Undertake weekly care home rounds, liaising closely with care home staff, practice GPs and colleagues to support residents.
* Support GPs by triaging and attending acute home visits in rotation with the established clinical team; manage acute illnesses in patients who are unable to attend the surgery, including arranging admissions to hospital or liaising with community teams, palliative care teams and social care.
* Deliver support to housebound patients to improve management of LTCs and help with decisions around ceilings of treatment and reducing hospital admissions.
* Provide a high standard of patient care using advanced autonomous clinical skills and a broad, in-depth theoretical knowledge base.
* Contribute to QOF/IIF targets for the Practices and support ongoing development of both the care home and visiting service.


About us

Wyvern Health is a Primary Care Network (PCN) of three GP practices: Priory Road Medical Centre, Lawn Medical Centre and Old Town Surgery. We are a high-performing group with a strong ethos of continuity of patient care, teamwork and peer support. We have a list size of circa 27,500 patients. Our clinical system is SystmOne and our most recent CQC rating was GOOD. The PCN has 10 GP partners and an extensive clinical team of advanced clinical practitioners, general nurses, pharmacists, pharmacy technicians, first contact physiotherapists, health care assistants, care co-ordinators and admin support staff, working alongside a management team and sharing best practice to benefit both our teams and our patients.


Job responsibilities

Clinical practice includes ward rounds at nursing/care homes to deal with clinical queries (some urgent to avoid admission), medication reviews, referrals and end-of-life care. Manage requests for acute home visits, triage appointments and visiting. Support the care of housebound patients. Ensure high standards of treatment and care for all patients (including complex patients, both acute and chronic). This involves diagnosis, implementation of care plans considering physiological and psychological factors, and ongoing evaluation to ensure patient compliance. Work closely with pharmacists to prescribe and review medications for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice, national and practice protocols, and within scope of practice. Promote health and motivate patients to improve general health. Communicate effectively with patients and carers/care home staff, recognising diverse communication needs and cultural backgrounds. Recognise issues and gather sufficient information to refer (e.g., drugs; domestic violence; child protection; vulnerable adults; social problems). Manage and assess risk within the areas of responsibility, ensuring adequate measures to protect staff and patients. Ensure work areas comply with health, safety and security legislation, policies, procedures and guidelines. Follow all necessary infection-control measures according to local and national guidelines.


Person Specification


Experience

* 5 years post-registration experience.
* Recent experience within primary, community or emergency settings.
* Ability to work under pressure.


Skills

* Communication skills, both written and verbal.
* Ability to assess a situation and obtain required support.
* Communication of difficult messages to patients and families.
* Negotiation and conflict management skills.
