THE PURPOSE OF THE ROLE
As a Case Management Assistant you will be responsible for providing day to day operational support to a team of Clinical Case Managers. This role would suit someone who has experience as a Personal Assistant, Executive Assistant or Project Assistant.
WHAT YOU WILL BE DOING AS CASE MANAGEMENT ASSISTANT
35 hours per week, Monday to Friday
1. Provide day to day operational support to an identified group of case managers.
2. Undertake research as directed for the sourcing of services, equipment and other client related requirements.
3. Initiate and coordinate the management of the third party provider process.
4. Undertake the administration and recording in relation to client funding requests for the above.
5. Provide monitoring support to the client account process ensuring case managers have access to accurate balance information when needed.
6. Assist clients to prepare for meetings and appointments
7. Liaise with members of the multi-disciplinary team as required.
8. Follow up request for GP and medical records
9. Undertake all required administration requirements as indicated by the identified case managers
10. Be responsible for the organisation of client related meetings, ensuring invitations are issued, agenda’s produced, accurate minutes maintained and circulated.
11. Be responsible for the gathering of pre meeting reports and summaries as required.
12. Undertake direct client related work as allocated by the case manager. This will include the establishing of timetables, implementation of agreed strategies and monitoring to report back on their effectiveness
13. Set up taxi accounts
14. Ensure contacts are added correctly
15. Booking taxis for clients
16. Flagging up when goal sheets and risk assessments need to be updated / reviewed
17. Ad hoc tasks as required
SKILLS AND EXPERIENCE REQUIRED
18. Excellent written and verbal communication skills including excellent client relationship management.
19. Microsoft Office proficient and high level of confidence using Apps and boking services.
20. Preferably having experience working with disabled or vulnerable individuals.
21. Excellent organisational and interpersonal skills.
22. Ability to work independently on a broad variety of projects with competing deadlines.
