This is an exciting opportunity for an Advanced Nurse Practitioner to support patients with urgent on-the-day care needs in the local community. This role will support the General Practitioners and the Multi-disciplinary Team to deliver safe, effective and person-centred care to patients in a primary care setting. You may work in GP practices, visit residents in care homes or attend to patients in their own homes. There is also a commitment to the Four Pillars of Advanced Practice, allowing for the development of skills in education, mentoring, audit, leadership, and quality improvement.

You will therefore be required to be working in an Advanced Practice role and have undertaken MSC (level 11) Advance Practice studies to PG Dip level or have equivalent experience in an advanced practice role.

You will require excellent communication skills and be able to work as a member of the wider Health and Social Care Team. They should have the ability to work autonomously, utilising their own initiative, clinical judgement and decision making to support the nursing workforce within the practices. This is an opportunity to join a dynamic team, with quality at the heart of all we do.

You may, on occasion, be required to work in an area other than those to which they are allocated.

It is ESSENTIAL that you have checked that you either already have an appropriate right to work in the UK or that the post would be eligible to be sponsored BEFORE submitting your application form.

Due to legislative changes from 1 April 2025, this post may require a different level of criminal records check done than is currently the case. If the post is assessed as a "regulated role", your appointment will be subject to joining the Protecting Vulnerable Groups (PVG) Scheme.
